We have exciting news to wrap up the work week! Commonwealth Senior Living has announced its partnership with Curana Health and Serviam Care Network through the Value-Based Care Alliance of Virginia: an initiative that provides seniors with access to in-residence primary care services and the option to enroll in Curana’s Medicare Advantage Institutional-Equivalent Special Needs Plan. Additionally, Commonwealth is participating in Curana Health’s Medicare Shared Savings Program Accountable Care Organization: a value-based care initiative that enables partners to earn shared savings by supporting care coordination activities and helping reduce unnecessary hospitalizations.
