It is said that problem known is half solved. It is pertinent to identify the problem either by holding one on one interaction with the team or personnel the issue has affected to really explain what went wrong and how it is used to be, what caused the deviation and what it ought to be (due to fact the problem when solved will alter what the solution used to be). Through the interaction, the problem will be clearly identified. Then devise methodology of solving the problem by assigning SMART tasks to those that are responsible for solving the problem who has also listened and inquired during the problem identification interaction sessions. All these processes must be documented, reviewed and accessible to the stakeholders for vetting.
