Determining the location or process stage "where" the problem occurred is essential for isolating issues. Pinpointing "when" the problem took place helps identify patterns. Investigating "why" or the root causes the problem is vital for addressing the issue at its source, not just its symptoms. Examining "how" the problem unfolded provides insights into its mechanisms, aiding in the development of effective solutions. Quantifying the problem, "how much", whether in terms of losses, defects, or costs, helps prioritize issues and allocate resources

I often employ the Fishbone Diagram, also known as the Ishikawa or Cause-and-Effect Diagram. Begin by clearly defining the manufacturing problem. This serves as the "head" of the fishbone. Draw a horizontal line extending from the problem's "head." Then, create branches (like fishbones) representing major categories of potential causes. These categories include People, Process, Materials, Machines, Measurement, and Environment (the 6Ms). Under each major category, add subcategories that might contribute to the problem. For each subcategory, brainstorm potential causes that could be linked to the problem. Prioritize potential causes based on their likely impact to the problem. Use the diagram to explore how different causes may relate.

I utilize the Corrective Action Report (CAR) method. Start by clearly defining the manufacturing problem. Collect all relevant data and information. Determine the root cause of the issue. Devise specific corrective actions. Clearly assign responsibilities to individuals for implementing each corrective action. Establish realistic timelines for the completion of each corrective action. Execute the corrective actions as planned. After implementing the corrective actions, verify their effectiveness. Document all the steps taken, including the problem definition, root cause analysis, corrective actions, responsible parties, timelines, and verification results. This comprehensive report forms the CAR.
