Just recently at the end of month pre-start meeting someone pointed out that there was only one red day on the month's safety cross. I brought up the point that it had been pointed out to me previously by another company that when you looked at the safety cross, ideally it would resemble a pizza. If your safety cross shows nothing but blue or no incidents it can actually indicate that the safety culture is one of avoiding reports of Safety incidents. Of course if there are numerous incidents or even near misses it is definitely an indication that there is something wrong and something needs to be done before either more or a severe incident occurs. I'm sorry but I completely disagree with this statement. I've seen too many companies in my experience who on paper look faultless. At the end of the day a company can provide you with the best training, have records of pre-start meeting topics, toolbox records, safety interactions but still push you to work in an unsafe manner knowing that if something goes wrong the supervisor can say that it was the workers fault for not stopping the job. Alternatively you could have a company with a terrible record or even a fatality who completely changed the culture in the workplace. It comes down to your individual Supervisor and more often than not they can practice a lot differently than they preach.

Depends on the size of the company. A small company may show “no citations.” Is this because they have not been inspected? A large company may have a long history of violations. Look for themes - lots of machine guarding? Lockout? Mobile equipment? Falls from elevation? Or more minor violations. Recent violations - or older. Without recent may show a company that has its act together. Maybe the company is hiring to to address their compliance troubles? I have taken jobs like that because the leadership discussed up front their challenges and plans to address.
