    What Is an Accountable Care Organization?

    An accountable care organization (ACO) is an organization of health-care providers that is responsible, both clinically and financially, for the care of patients across settings, from physician offices to acute care to long-term care facilities.

    ACO contracts reward providers for improving the quality of care and lowering the total cost of care. ACOs are loosely managed, with no gatekeeper referral or preauthorization requirements.

    The major ACO program today is the Medicare Shared Savings Program (MSSP). Many commercial insurers are also pursuing ACO programs to reward providers for improving care quality and lowering costs.

    MSSP ACOs are either of the following:

       Physician-only ACOs—group practices, networks of individual practices (individual practice associations), or selected federally qualified health centers and rural health centers

       Hospital–physician ACOs—either hospitals and physicians in partnership or hospitals with employed physicians

    ACO entities must
