Another question - in dementia care homes how common place is it for breakaway or deescalation techniques & training in this to be used even as a last resort - or has person centred care implementation proven that there is no place in new humanity rights based cultures for this ?

In 2022, the British Psychological Society (BPS) published guidelines for conducting psychotherapy with men, marking an important shift in the field. This initiative by the BPS acknowledges the need for practitioners to rethink their therapeutic approach when working with male clients. Instead of simply urging men to "go to therapy" and expecting them to share emotions openly, these guidelines address a key issue: many men may struggle to articulate their feelings in the ways traditional therapy models assume. Over recent years, psychotherapy has increasingly recognized this challenge, prompting research and discussion about how to make mental health services more effective and accessible for men. As a result, the field is evolving, asking critical questions about how to better support men’s mental health and to create therapeutic spaces that truly engage them. Societal pressures frequently discourage men from showing vulnerability, leading to untreated mental health issues. For instance, according to the Harvard Gazette while 1 in 10 men experience anxiety or depression, fewer than half seek treatment. Many feel a need to “hold it together” rather than reach out for support​. Here are some key takeaways: 1. Education and Orientation: Men often benefit from an upfront explanation of therapy, symptoms, and goals. This can help reduce uncertainty about the process and increase engagement by addressing initial discomfort about therapy​. 2. Action-Oriented and Goal-Focused Therapy: Many men prefer a practical, goal-oriented approach. Integrating active problem-solving—such as session goals and progress reviews—can align with this preference and help them feel a sense of progress​. 3. Emphasizing Collaboration and Transparency: Building a collaborative and empowering therapeutic alliance is essential for men. This approach fosters autonomy, which can be crucial in reducing the likelihood of early dropout. 4. Positive Masculinity and Language: Integrating positive aspects of masculinity, like responsibility and loyalty, can help reduce stigma and encourage more genuine engagement. Additionally, using straightforward or de-stigmatizing language can make therapy feel more accessible​.

'While identification with the analyst’s capacity to entertain and tolerate multiple perspectives may contribute to a process that ultimately leads to the patient’s development of a capacity for reflective thought, identification, in and of itself, is not likely to be sufficient to bring about such psychic change. The analyst needs to work to interest the patient in the broad workings of the patient’s mind, rather than relying on traditional interpretations that elucidate unconscious content. He needs to persistently draw the patient’s attention to behavioral and psychological phenomena indicative of underlying psychic processes, particularly when the patient has deemed such phenomena meaningless or, alternately, fully explained by the patient’s beliefs about the phenomena. #ADHD #diagnosis #drugs #despair The last few years has not been kind to people with #ADHD or those seeking assessment. Increasing awareness that this common #neurodiversity condition persists into adult in 50-60% of cases prompted increased referrals, combined with #lockdown #medication shortages & defunded #mentalhealth services leaving people with a very treatable condition frustrated & unable to move forward & fulfill their potential. It’s particularly hard to tell someone with ADHD to be patient & we shouldn’t have to. Decades of mislabelling, masking + misdiagnosis are common in many of my patients. Elevated rates (especially of undiagnosed persons) are common in #drug #dependence & #prisons. These marginalised groups tend to have poorer access to treatment. This is not good medicine & makes poor economic sense. Effective treatment can help people return to easier happy healthier lives benefiting themselves, their family and economy. Staying Safe V3 out Sept has 3 modules on neurodiversity. Helping people reflect on their health, their behaviours to make informed choices about treatment and lifestyle. Treating ADHD is one of the most rewarding parts of my clinical work. Better training, earlier diagnosis & treatment of the most vulnerable also makes good economic sense.
