    Information and communication technologies (ICTs), such as mobile communication networks, and behaviour-based approaches for citizen engagement play a key role in making future cities sustainable and tackling persistent problems in high-density urban areas. In the context of Sharing Cities, an EU-funded programme aiming to deliver smart city solutions in areas such as citizen participation and infrastructure improvements of buildings and mobility, a prominent intervention has been the deployment and monitoring of a Digital Social Market (DSM) tool in Milan (Italy). The DSM allows cities to engage with residents and encourage sustainable behaviours by offering non-monetary rewards. This paper aims to evaluate the effectiveness of the DSM approach to promote active travel (cycling and walking) by analysing the data collected through the app as well as through participant surveys. Our model results show that a broader engagement with the DSM app (number of claps to posts, number of posts made, non-monetary rewards earned by participating in non-travel events) is positively correlated with the monitored level of active travel. Lifestyles, attitudes, and social influence also explain the variability in cycling and walking. This highlights the importance of investigating these factors when replicating such initiatives on a large scale.

    The Digital Social Market (DSM) is a service which incentivises citizen engagement amongst the context of sustainably oriented city services. The approach aims to shift perceptions and behaviours towards city sustainability goals by providing reward mechanisms in exchange for continued and enhanced citizen engagement. There are many forms and examples of incentive mechanisms in existence; however, this approach is unique as it aims to form a value chain which places social and citizen needs first; the DSM is driven by the needs of the city, of fostering meaningful connections with citizens to catalyse change, not profit. By adopting this approach, the outcome is a set of services that aim to facilitate a stronger connection between city and citizen, empowering the latter to take an active role in participating in the collective future of their urban spaces. The DSM framework has been designed and implemented in partnership with three of the most advanced European lighthouse cities: Lisbon, London and Milan. In each location, the project has adopted a different focus such as mobility, domestic energy management or community cohesion. In addition to this, each city model includes a different social component such as schools or local charities, who also benefit from citizen involvement. This social dimension has brought significant complexity but also a great deal of learning around the value of community engagement in the development and deployment of smart city services. The approach to the DSM includes an online and offline component, both of which have been developed through an array of service design methodologies. This approach has enabled the project partners to start at the very beginning of the challenge, co-creating each aspect of the service together. The DSM is part of the European Commission Horizon 2020 funded programme Sharing Cities.

    Objectives: To evaluate the impact in terms of use of health services, clinical outcomes, functional status, and patient´s satisfaction of an integrated care program, the CareWell program, for complex patients with multimorbidity, supported by information and communication technology platforms in six European regions.

    Objectives: To evaluate the impact in terms of use of health services, clinical outcomes, functional status, and patient´s satisfaction of an integrated care program, the CareWell program, for complex patients with multimorbidity, supported by information and communication technology platforms in six European regions.

    Data Sources: Primary data were used and the follow-up period ranged between 8 and 12 months.

    Study design: A quasi-experimental study, targeting chronic patients aged 65 or older, with 2 or more conditions – one of them necessarily being diabetes, congestive heart failure or congestive obstructive pulmonary disease. The intervention group received the integrated care program and the control group received usual care. Generalized mixed regression models were used.

    Data collection: Data were obtained from individual interviews and electronic clinical records.

    Principal Findings: Overall, 856 patients were recruited (475 intervention and 381 control). In the intervention group, the number of visits to emergency rooms was significantly lower, and the number of visits to the general practitioners and primary care nurses was higher than in the control group.

    Conclusion: The CareWell program resulted in improvements in the use of health services, strengthening the role of PC as the cornerstone of care provision for complex patients with multimorbidity.
