All you need is an explanation? 🗣 ❓   I enjoyed the latest joint RSM Digital Health Section and @Dev&Doc podcast featuring Dr Annabelle Painter, Dr Josh Au Yeung, Zeljko K. on #XAI.   𝐓𝐡𝐢𝐧𝐠𝐬 𝐦𝐞𝐧𝐭𝐢𝐨𝐧𝐞𝐝 𝐭𝐡𝐚𝐭 𝐈 𝐟𝐨𝐮𝐧𝐝 𝐢𝐧𝐭𝐞𝐫𝐞𝐬𝐭𝐢𝐧𝐠: 🧠 We need to trust a solution to accept it's decisions, and part of trust is understanding why - therein lies why we find explainability so important but actually it really boils down to our ability/willingness to trust?   🧠 We frequently use the terms ‘#explainability’ and ‘#interpretability’ with respect to AI as interchangeable, but they mean 𝘥𝘪𝘧𝘧𝘦𝘳𝘦𝘯𝘵 things (see: https://lnkd.in/eKuNMbXx.)   🧠 When we talk about explainable AI – do we mean explainable for each decision or how the system as a whole is working or both? And are they equally important? Also, could we even understand it or find it relevant (e.g. complex mathematical reasoning).   🧠 Whilst we expect AI to be explainable, is this a double standard when we don’t even know sometimes how humans make decisions? 🤷‍♀️ When referring for specialists/tests sometimes we have ☑ that says ‘clinical concern’ for when we can’t explain why a patent needs help – so if we can’t always explain our own intuition and make allowances for that, maybe we need to for AI? 🤔   💡 Fascinating point re: whether having an explanation makes doctors more likely to accept a false result. So could this actually worsen our over-reliance to AI? 😨  Something I saw raised in a Nature letter here published on explainable AI in dermatology. Lauren Oakden-Rayner gave a lecture several years back and discussed a 2015 study about how pigeons 🐦 could detect breast cancer on pathology images (https://lnkd.in/eD3beHN2 😂 ) and argued how they could be seen as a kind of ‘model’ outputting binary classification with high accuracy but we’d never dream of thinking of a 🐦 = 👩, yet we do this comparison with AI.   🤖 The topic of human-AI interaction and decision making cropped up a few times as an area of much needed research – although I’m certain there must be a lot out there given all the courses/degrees/jobs related to #HCI and #UX? No?   Anyway - lovely episode! Would enjoy hearing from anyone working on exciting projects that cross behavioural & social science / medtech. Always a fan of Freakonomics and how predictably unpredictable we can be with decision making…!

Today I was invited to join a panel to discuss Digital Technologies in Life Sciences as part of the National Health Executive's (NHE) NHS Life Sciences online conference. In the discussion with my great co-panellists we explored how digital technologies can play a greater role in improving research, accelerating new treatments, enhancing care delivery and enabling greater self care. The potential opportunities and challenges of greater levels of self care through technology was the topic we explored in most depth. Insights included: - This is a contested space with a multiplicity of actors, technologies, business models and ethical issues that are still being worked out. - There is significant additional value to be realised for people/patients, healthcare systems and industry if we can find the right systemic solutions. - There is real a risk however, that we use technology to extend legacy health silos into digital silos driven by health conditions rather than addressing the more whole person needs of people and patients. - Care needs to be designed around the more holistic needs and lived experience of people, families and communities. - Digital thinking therefore needs move beyond "digitising what we do" to "re-imagining what we do in a digital world" - Digital health is different from digital services in other industries in that the value created is not through what happens on the screen but how a person's offline behaviours that can be changed and sustained over time. - This requires much deeper understanding of the psychology of behaviour change and the a shift from a "telling/instructing paradigm" to a "coaching paradigm" which most current digital health solutions are missing. - Any future health passport and/or NHS App strategy will need to embody the above insights if it is to go beyond a place for storing health data to a valuable and meaningful aid in enabling greater self care across all parts of society. All in all a valuable and stimulating conversation that helped to shine a light on some of the factors that will be key to enabling digital services to play a deeper role in improving our future health and care. For all the health people in my network, please fill in this survey. Explain to NHS England that this is a terrible idea that will create further barriers to innovation when we need it most. The UK can be a powerhouse in medtech innovation but not like this. Empower the frontline to seek innovations that solve the real world problems they face. Award contracts for real implementations not pilots and trials or schemes. A new report by the BBC shows how quickly the adoption of AI is happening in clinical settings. It's just a matter of time where this becomes the norm. Here's one use case where it makes perfect sense to integrate AI. The National Institute for Health and Care Excellence (NICE) in England is moving towards recommending AI tools that help detect fractures in X-rays. These tools aim to address the frequent issue of missed fractures in emergency departments, which can be as high as 10% of cases, due in part to staff shortages and heavy workloads in radiology. By using AI to assist in initial fracture detection, clinicians can potentially speed up the diagnostic process, reduce follow-up appointments, and ultimately improve patient outcomes. However, AI won't replace the expertise of healthcare professionals. Instead, it will act as a second set of eyes, with a radiologist reviewing each AI-assessed image. This approach is part of NICE's cautious, evidence-based integration of AI in healthcare, ensuring patient safety and diagnostic accuracy.

Check out this rooftop view at a recent Cambridge Health Network event. Pam Garside and Penny Dash set-up the CHN 20 years ago, arranging a consistent cadence of fantastic events with stellar healthcare leaders discussing the most vital topics at the best venues across London. Here are my takeaways from their most recent event featuring senior execs from NHS Trusts across London: 🍎 5% of patients utilise 50% of the total primary & secondary healthcare budget - our priority has to be addressing the needs of this 5% (source: https://lnkd.in/efZBbnsN) 🍎 EHR implementation is the start of the journey for a trust, not the end point 🍎 In the US there are several dedicated EHR assistants who support clinicians to maximise the utility of EHRs 🍎 Barts Health NHS Trust has demonstrated that virtual wards are effective - they help save costs, and free up space for more inpatients to be treated

Last week InsideOut® was selected via a competitive pitch process to attend Innovate UK’s Health Tech Investor Readiness event organised by the business growth team. We delivered a 5 minute pitch to both angels and VCs who provided constructive feedback on our pitch and pitching style. Key takeaways from our pitch: 1. We have a good deck and clear slides 2. The business is uniquely solving a real world problem in what is a busy space 3. Revenue is key in this climate. A well thought through model with a clear journey of how we will achieve our targets is essential. You might have seen in the media that the team at Flok Health have launched their AI physio service, enabling patients with back pain to have an AI-generated video consultation. Thinking back to pre-covid, the prospect of the NHS introducing video consultations to mainstream services is unimaginable. Now in 2024, they are piloting the use of AI video consultations! What are your thoughts? Is this is a risk to health care, or a step towards the future? What other ways do you think we will see AI being used in the future (or what ideas/opportunities are there)?

Your regular reminder of the importance of inclusive data stewardship. Examples such as the below reveal how far we have to go until data governance truly works for everyone. And of course the implications are significant, given the acceleration of AI technologies driven by data. Initiatives like Biobank rely heavily on social contracts and data donation, and careful thought and consideration about how data sets might be used. Can LLMs be truly explainable? What are the methods we can use to understand how a model works? Is explainability a essential requirement for Clinical AI? 🤔 🔹This episode was a merging of Dev and Doc and Dr Annabelle Painter from Royal Society of Medicine's digital health section podcast. (it was only a matter of time for these two great podcasts to join forces 😉) 🔹We sit down to deep dive into the fascinating topic of explainable AI, methods to interpret models, research on how we can understand deep learning models and LLMs. 🔹 As usual Dev Zeljko K. gives us an amazing technical overview of old and modern approaches including heat maps, pertubations, LIME techniques 🔹We highlight the importance of behavioural science of the clinician AI relationship, how that can lead to automation bias and "placebic information". 🔹It was a lot of fun and a pleasure to sit down and chat together. In the Labour Party’s June 13 manifesto, the party committed to setting spending plans for the next decade, providing funding to research agencies like UK Research and Innovation (UKRI), the UK's primary public science funder. Departing from the usual three-year budgeting cycle, the manifesto aims to foster long-term collaborations with industry to maintain the UK's leadership in global innovation. The Association of the British Pharmaceutical Industry (ABPI) promptly congratulated Keir Starmer and the Labour Party on their General Election victory. The ABPI's statement, released shortly after the election results, reflects industry optimism and support for the new government's initiatives.
