Insightful report that clearly articulates the current challenges

It was inspiring to talk to Dr Miia Turpeinen - the hospital director of Oulu University Hospital - for the latest issue of FORTIS magazine. Dr Turpeinen has been intricately involved in the €1.6 billion project to build the world's smartest hospital in Northern Finland, which will be fully operational in September. She says: "We make the bold claim that it will be the smartest hospital in the world when it is fully operational in September this year. Yet it is not just about new technology, although of course that’s an important part of its functioning, but our vision is also focused on creating more patient-centred care and better meeting the needs of our staff so we can boost retention. To do this we had to change the way we do things. In the past, we would have implemented these changes with old-style thinking - namely that we, as leaders, know what is best for our patients and staff. But this time we approached it differently, actually asking the patients, staff and students what they needed and wanted in over 500 specially designed workshops.  We consulted them about treatments, processes and the design of the new hospital. We have built the hospital through cooperation using this model from builders funders and users - everybody has participated together so that it’s a mutual win-win. If there is a problem it is everyone’s problem, and we solve it together, and look for solutions, rather than blame each other. I couldn’t imagine building a hospital the old-fashioned way again." The Finnish healthcare system faces many of the same challenges as the NHS, including staff shortages and the needs of an ageing population. However, this project offers useful lessons not only about the importance of technology but also about working holistically and humanely. Earlier this week, a report on the Care Quality Commission's operational effectiveness was published, leading Health Secretary Wes Streeting to call for an overhaul of not just the CQC, but all other bodies responsible for patient safety in England. Led by Dr Penny Dash, the report highlights the CQC's operational failings, which include a lack of inspections taking place, problematic IT systems, and inconsistencies in assessments. Our article provides a summary of Dr. Dash's findings and recommendations, with a particular focus on the criticisms of the CQC’s single assessment framework and the need for reform. 📣 OUT TODAY Bright Blue's new report on reforming the NHS by empowering Integrated Care Boards (ICBs). It is authored by Bright Blue Associate Fellow Conrad Bannister. The NHS in England is in trouble. It is underperforming, both historically and internationally. But the government needs to prioritise more reform, not more money, to improve the performance and efficiency of the NHS. ICBs have an opportunity to improve the way the NHS in England operates, if they are given the finances, outcomes and governance changes to realise their transformative potential: to build an NHS in England that is more local and long-term, focussed on prevention and patients and care closer to home. This report offers 12 policies that would support ICB reform in this country, as well as enable the NHS to alleviate the problems it faces. The report’s recommendations flow from four key principles: 🔴 Prioritisation of the long-term. The report seeks to avoid short-termism. The following policies are not designed to fix immediate problems, but making the NHS sustainable and efficient in the long-term. 🔴 Fiscal responsibility. The report recognises the tight fiscal constraints the current Labour Government is under. Current public expenditure in the NHS is significant and England is currently not receiving good enough healthcare provision for the money that is spent. Therefore, the policy ideas in the report will not generally be to spend significantly more public money. 🔴 No top-down structural reform. The current Secretary of State for Health said that “[Labour’s plan] isn’t about changing structures.” There is a huge amount of goodwill towards ICBs: they are supported across the political spectrum. 🗣️Darren Hughes: “With just over 18 months until the next Senedd election, the pressure is on for the new cabinet secretary to enable the healthcare system transform services and improve outcomes for patients across Wales." The six key enabling priorities for NHS leaders in Wales are: 1. A government-led national conversation on how the health and care system can innovate and transform to meet the needs of future generations. 2. A strong central steer on service redesign, supported by simplified local consultation. 3. A ten-year capital investment plan to reshape NHS estates and infrastructure (including digital), authorising new capital funding models to enable investment. 4. Providing certainty on long-term funding including a long-term planning framework for financial recovery. 5. Workforce: Making the NHS a more attractive place to work and build a career to bolster the NHS’s ability to recruit and retain its workforce, and 6. Shifting resources towards preventative activity across the whole public sector, progressing towards One Welsh Public Service.
