The recent Lord Darzi report recognises the NHS is in dire need of a #digitaltransformation. The report highlights how our nation’s health service is in the most perilous position since its inception in 1948. Core recommendations include a move from hospitals to community care, and a greater emphasis on prevention and training. Solutions offered include:  ✅️ The transition from analogue to a digital NHS   ✅️ Moving more care from hospitals to communities   ✅️ Focusing efforts on prevention over sickness    Reflecting on the simplicity of the original 1948 concept for a National Health Service, it places the family doctor as central to delivery. We believe #VR training for GPs and support staff is crucial in addressing the NHS training crisis. Here is an example of exactly that. In my recent article for Digital Health, I make the case for digital innovation as a driver for change, using our Sussex Community NHS Foundation Trust Digital Innovation Lab as a case study. Some may see innovation as a 'nice to have', especially difficult to prioritise when budgets are tight. I disagree. I've just googled the definition of innovation and it says it's the introduction of new ideas, methods, or devices, or the practical implementation of ideas that result in new or improved goods or services - don't those sound like things we should be doing all the time? Innovation isn't just about shiny new technology and expensive gadgets (although it can be). For us, it's about equipping our workforce to think differently and be empowered to implement their new ideas for the benefit of patients and staff. They often just tick📝 a box, and do little more than that. Clinical Governance Frameworks (CGF) play an important role in any health organisation and shouldn't be overlooked. Here is what I often see missing from CGF documents; 1. Specific metrics for what % of audits will be completed and how frequently (also what is then done with the results). 2. How complaints and incidents are handled. 3. The operating rhythm for the CG committee. 4. Where incidents and risk registered are stored and how they are treated. 5. Where minutes and actions of committee meetings are stored. 6. How any representatives (especially when consumer engagement is involved) are selected. 7. Targets for key metrics cited such as consumer/clinician engagement. 8. How the credentialling of staff is managed. ❓How does your CGF stand up? ❓What do you have in your organisations that really works for you? @impacthealthcareconsulting

I’ve read a couple of very useful papers from NHS Confederation and National Association of Primary Care (NAPC), which are relevant to Mutual Ventures Radical Place Leadership approach (links at the end). The development of place policy and Integrated Neighbourhood Teams are hot topics at the moment. NHS Confed says the current financial environment is driving systems in the opposite direction they (reportedly) want to go in. New Public Management approaches remain the easiest/simplest thing to reach for when finances are tight. In my experience, some system leaders see empowering places and neighbourhoods as risky or a "distraction"… Another challenge can be the lack of a shared purpose. The one thing local public services don’t lack is a plethora of strategies and plans - but they are seldom aligned. NAPC highlights the importance of a shared purpose for successful integrated working. In our Radical Place Leadership approach, looking at what exists and developing a single vision / set of priorities is the first thing we do. NHS Confed also explores differences in how systems approach place and neighbourhood working. The “Russian doll” approach where there is genuine decision making and delivery devolution (very good) – and then the hub and spokes approach where neighbourhoods are simply the delivery arm for a plan set at a system level (not so good). NAPC emphasises the importance of relational ways of working which is welcome - but easier said than done in systems where New Public Management is still the norm (see Mark Smith's work on the Liberated Method). The Integrated Neighbourhood Model where you have a core team with a wider set of specialists who can be drawn in, is supported by NAPC. When this model is driven from a health perspective, most of the roles (and ambitions) tend to be clinically focused. our Radical Place Leadership approach involves a wider group such as police, DWP/Job Centre plus, schools and housing – essentially all the support a particularly vulnerable person might need to get back on track. The NAPC report also talks about how you identify the people who need support. We have been working with Georgina Cox on our solution to this. The truth is it can be as simple as reaching out to the people you observe on the street who clearly need help, through to each partner organisation bringing a list of the people they are most concerned about, all the way through to complex risk stratification models. National approaches to digital transformation in NHS & social care are overly focused on picking the next tech winners without considering how to accommodate, shape or shift entrenched incumbents.

NHS trusts have some way to go on #digitalmaturity, the new snapshot report examining the Digital Maturity Assessment (DMA) scores of NHS trusts across England from Digital Health reveals. Out of seven success measures, ‘Ensure smart foundations’ emerged as the top-performing measure across all trusts, but with an average score of 3.2 out of five.
