    🌨️ Cold weather, warm actions: Help those sleeping rough this winter As temperatures drop across the UK, many people face the harsh reality of sleeping rough. It’s a reminder that homelessness is not just a statistic, it’s a deeply human challenge. At Unboxed, we partnered with StreetLink to transform how the public can connect rough sleepers with local support services. In just four months, we work with StreetLink to revamp the platform, leading to a surge in alerts: ➡️ 100,000+ alerts received since launching enhanced platform on September 30th 2023. ➡️ Over 16,000 individuals connected with support since launch. This user-centred platform is empowering communities to make a difference, ensuring no alert goes unnoticed and every effort helps people find the support they need. 🔗 How you can help If you see someone sleeping rough, you can make a difference in minutes. Use the StreetLink platform to notify local outreach teams, providing them with the information needed to connect someone with vital help.   • 🚀 Transforming healthcare through patient-led innovation Guy's and St Thomas' NHS Foundation Trust partnered with Unboxed to launch a patient-led innovation programme, uncovering over 20 opportunities to enhance care delivery. By engaging directly with 50+ patients, we tackled key pain points in services like Rheumatology, Dermatology, and Children’s Allergy, introducing impactful solutions — including a remote monitoring service for rheumatoid arthritis patients in South London.   • 🌲 Our Unboxed christmas party was criminally good! We had an absolute blast at Alcotraz, where our problem solving (and acting) skills were put to the test in the most unconventional, and hilariously fun, setting. After serving our "time," we ditched the orange jumpsuits for something more festive and sparkly, heading to a celebratory dinner to toast a year filled with success, teamwork, and innovation. 🥂 Here’s to another incredible year of making an impact together.   • 🎉  That’s a wrap on our virtual roundtable: The untapped potential of PROMs! A huge thank you to our incredible speakers Melanie Martin, Laura Smith, and Dr Markus Gwiggner for sharing their expertise and sparking thought provoking discussions on how Patient Reported Outcome Measures can be implemented to improve healthcare. As well as Rhian Lewis and Tom Gayler for facilitating and summarising points of discussion. We heard about the value of integrated PROMs in service transformation, including creating space for tackling inequalities of access. We also discussed how PROMs enable new forms of relationship between healthcare providers and patients both in acute care and general practice, empowering people to take a role in their healthcare as long as providers were brave enough to take the plunge!   • ✨ Transforming patient care with remote monitoring We’re excited to share our work with Guy's and St Thomas' NHS Foundation Trust, highlighting a groundbreaking remote monitoring service developed for NHS patients. Through user-centred design, we’ve created a solution that empowers patients to self-report symptoms from home, improves care between appointments, and streamlines clinical workflows. Key highlights: ✅ Patients feel more cared for between appointments ✅ Clinicians can better track and respond to changes in symptoms ✅ 70–79% completion rate for online patient-reported outcome measures (PROMs). The challenge Managing chronic conditions like rheumatoid arthritis often involves regular hospital check-ins, which can be inconvenient for stable patients and frustrating for those struggling to get timely appointments. We saw an opportunity to improve the process with a simple, scalable solution. But Laura Smith and Tom Gayler are challenging this idea in their talk ‘Going beyond viable’. They draw on their experiences of  creating change in the UK planning space to think about the role service designers have in shaping the environments in which services exist and are created. They share their experiences of shifting perspectives and how adaptability and self-reflection enabled their practice to evolve and to deliver services which flourish.
