Congratulations to my old TRL colleague Goff Jacobs on the award of an MBE in the King's Birthday Honours list. It's always useful to review decisions around risk - in this case, nobody took account of the fact that cars are now wider, and so are parked closer together in spaces which didn't get any wider, plus had an increasing amount of plastic Vs metal making it easier for fire to spread. In work on hostile vehicle mitigation, we'd often find that nobody had recorded a particular tree or other physical feature as part of what was protecting a location, in a way which meant that any changes would be flagged up as needing alternative mitigation. It's the same with road safety. Whether it's a strategy document, a policy or a procedure, it should always have periodic review, ideally with some input from someone with a fresh pair of eyes and not just the document authors, because it can be difficult to spot issues in documents or systems to which we are too close (which is why proofreading is important!). https://lnkd.in/eMjXadBx

It's not every day that Leicester reminds you of Abu Dhabi... But yesterday that's what happened. I was at the National Space Centre presenting at an excellent day put on by National Highways and the Driving for Better Business team, which was both informative and challenged traditional thinking (I even got to have a bit of a to-and-fro discussion with Matthew Syed, although in the end it turned out we actually agreed with each other, so I avoided being challenged to a game of table tennis in the car park to settle the matter). The conversations reminded me of Professor Rhona Flin's excellent paper to the Society of Petroleum Engineers conference I presented at in Abu Dhabi back in 2006 "Managerial Resilience and Safety: Vasa to Nasa" which forms a chapter of the book "Resilience Engineering - Concepts and Precepts". The essence of managerial resilience is the ability to manage severe pressures and conflicts between safety and the primary performance goals of the organization. Managerial resilience requires risk assessment, decision making and assertiveness skills - operational and safety goals can often be seen as competing, requiring the manager to make ‘trade-off’ decisions - "get the job done safely" all too easily becomes "get the job done" when there is a conflict between the two. Professor Flin examined notable organisational failures - the sinking of the Swedish warship Vasa 1624; the space shuttles Challenger 1986 and Columbia 2003, and the oil platform Piper Alpha, 1988. In all cases, the erosion of managerial resilience appears to have played a key role. It really is worth a read, and if you haven't come across it before I commend it to you, and it might be worth giving a copy to your board. Yesterday was a reminder that there is so much we already know we should and could be doing, but so much we still have to learn from each other on doing things better. Although all too often, we aren't getting the proven basics right - as someone rightly said to me over lunch, ironically given the venue: "I mean, it's not rocket science is it..." So Leicester - as thought-provoking as Abu Dhabi, but without quite such good weather.
