North Central Health Alliance has launched a new approach to care for people with complex long-term health conditions! Read the recent blog by Kate Petts and Rachel Lissauer to find out how this new programme will help to improve patients’ outcomes and experience of care through closer collaboration amongst primary care, secondary care and community services.
Despite having numerous appointments with different specialists many patients with long-term conditions feel "off the radar". They often experience fragmented and inefficient care, though account for over half of GP time. To address this and ensure these patients get the co-ordinated care they need, NCL Health Alliance are testing a new approach through the Complex Long Term Conditions Service. In this new blog Kate Petts and Rachel Lissauer explore the challenge around long-term conditions and how the new programme aims to tackle this by bringing together community, primary care and acute care. During the feature, Christiana Edwards-Aganmwonyi, one of our Clinical Support Workers, shared her experience of working night shifts at the Crisis Hub: “My role is ultimately working out how to provide support. It’s about reassuring the caller first and foremost, then actively listening to what they are saying and also what they are not articulating, then providing the appropriate care." 🌟 Driving Change in Cancer Care 🌟 A few days ago, the North Central London Cancer Alliance hosted the first ever North Central London Cancer Conference. Over 200 colleagues came together - from hospitals, primary care, the voluntary sector, commercial partners, charities, academia and more - to talk about how we're doing in the fight against cancer. Our CEO Frances O'Callaghan gave an introduction to the context of cancer across Barnet, Camden, Enfield, Islington and Haringey. We took the opportunity to acknowledge some great progress (for example our survival rates in certain forms of cancer are the best in the country). As Frances sets out, though, there's more we need to do - particularly around reducing waiting times, patient experience, and reducing inequalities. We were delighted to be at Central and North West London NHS Foundation Trust’s Community Services Open Day, an inspiring showcase of the vital work that supports patients beyond hospitals. 🌟 It highlighted the dedication of clinicians and frontline workers who deliver essential care directly in the community—making a real difference in the lives of patients. Engaging with those at the heart of these services underscored the importance of working together to enhance patient outcomes, and was a valuable reminder about how we can support patients to get the best possible care, in the best possible place for them, as an individual. Partnership working is key to delivering effective community care, so it was great to acknowledge and talk about the role of others and their teams too, such as North London NHS Foundation Trust, Whittington Health, North Middlesex University Hospital NHS Trust and Camden Council to name but a few. 🌟 Reflecting on an Inspiring Cancer Conference 🌟 On Tuesday, the North Central London Cancer Alliance hosted the first ever North Central London Cancer Conference at the Emirates Stadium. It was a packed day – with over 200 colleagues joining – from hospitals, primary care, the voluntary sector, commercial partners, charities, academia and more. The day was filled with engaging discussions, thought-provoking breakout sessions, working towards our shared commitment to improving cancer outcomes. We took the opportunity to acknowledge some great progress, but also talk about what more we need to do. For example, in lung cancer we have almost doubled our 1-year survival rate – nearly 6 in 10 patients (58%) survive the first year after diagnosis – nearly 10% higher than the England average – and up from 3 in 10 in 2005. Over a quarter (27%) of patients are now living for more than five years after diagnosis – compared to just 10% in 2005. But… as we heard from speakers including Ali Malik, Frances O'Callaghan, Prof. Derralynn Hughes, Prof Geoffrey Bellingan, Dr Clare Stephens, Caroline Clarke, Pete Landstrom, Jodie Moffat, Neal Navani (and a whole host of others leading breakout sessions…!) …there is still more we can and need to do. Particular focuses need to be to reduce inequalities, improve patients’ feedback on their care, and improve our performance on cancer waiting times. We also were delighted to welcome Amanda Pritchard, CEO of NHS England, who took the time to acknowledge our progress, and highlight what an opportunity there is to go even further. A big thank you to everyone who contributed to making this event such a success — from our inspiring speakers and session facilitators to all the attendees who shared their insights and ideas.
