📢 Celebrating our work in 2024 As we are nearly at the end of 2024, we are reflecting on our work this year. 💡 As part of our role to assist in the identification of the best solutions for our health and care systems across the region, we help to demonstrate the benefits of new services and products through real-world evaluations. We supported Lenus Health's funding application to the Small Business Research Initiative (SBRI) to implement its digital solution in Hull University Teaching Hospitals NHS Trust. The project analysed the effectiveness of Lenus Health’s solution in improving the management of patients with Chronic Obstructive Pulmonary Disease (COPD) and in reducing adverse effects, especially emergency admissions. We have a broad set of programmes which focus on achieving the following objectives: 1) Contribute to improving population health across the Yorkshire and Humber region. 2) Support service improvement, transformation and delivery of new models of healthcare. 3) Generate regional and national economic growth by creating strong partnerships between the NHS, HEI and industry. We achieve our goals by connecting the NHS, Higher Education Institutions and Industry to accelerate the process of innovation and facilitate the adoption and spread of innovative ideas and technologies at pace and scale. For the first time, we hosted the South Yorkshire Innovation Showcase. 💪🏼 This event in collaboration with the South Yorkshire Integrated Care System and delivered by our South Yorkshire Innovation Hub, was a celebration of the fantastic work happening across our region. 📢 Celebrating our work in 2024 💊 The national polypharmacy programme focuses on #improvingpatientoutcomes by highlighting the value of Structured Medication Reviews (SMRs) SMRs are a conversation between a #patient and their #clinician about their medication to make sure what they are taking is still the most appropriate for them, ensuring they get the best out of their medicines. Across our region, the polypharmacy programme has been instrumental in supporting frontline primary care staff to improve #medicationmanagement by increasing the uptake and use of the resources available through the programme. More than 80 clinicians across Yorkshire and the Humber have undertaken polypharmacy action learning set training available through this initiative to enhance their knowledge and skills. ✨ A year in review ✨ Commissioned by the Accelerated Access Collaborative, we developed an innovation framework, designed to encourage stronger and more effective collaborative working between all stakeholders involved in developing innovation adoption action plans. 👄 As part of the project, we interviewed patients with lived experience of using one of five innovations that are part of the MedTech Funding Mandate and Rapid Uptake Product programmes and associated care pathways, which are captured in the report ‘Driving Innovation Uptake Through Patient Insight’. ▶️ https://lnkd.in/eTP5M22X These insights have informed the development of a framework, which includes practical steps for those seeking to accelerate the adoption of innovations - ‘A Framework for Driving Innovation Adoption Through Patient Insight’. ▶️ https://lnkd.in/eRjvCcg6 The framework was built by aligning what matters to patients with actions that innovation leads can take at various points along the care journey. We’re joining the Yorkshire and Humber Good Work, Health and Skills Summit today in Leeds discussing how we can work collectively to support people to stay health and in good employment. 🏅 We’re proud to champion these efforts. This year, through our YHealth for Growth partnership with Yorkshire Universities and the NHS Confederation, we’ve been highlighting the vital connection between health and work. Yorkshire and the Humber faces a significant challenge, with the third lowest employment rate in England and 2.8 million people out of work due to long-term sickness. That’s why we’ve been convening regional partners to share best practices and develop solutions — from supporting individuals with ill health back into work to helping those currently employed maintain their health and stay engaged in fulfilling careers. ✨ Yorkshire and the Humber is perfectly positioned to pilot innovative, devolved solutions that address health and economic disparities. By working together across sectors, we can build pathways to meaningful change and create opportunities for everyone to access good work. 💪🏼 The Innovation for Healthcare Inequalities Programme (InHIP) has shown the power of local collaborations to tackle national challenges. Over 12 months, the Innovation for Healthcare Inequalities Programme (InHIP) has supported local teams across England to address healthcare inequalities. By focussing on the Core20PLUS5 priority areas, over 34,000 underserved individuals have benefited from improved access to healthcare innovations. Key achievements include: 🟠 Boosting cancer screening uptake in underserved communities 🟠 Delivering cardiovascular health checks in community hubs 🟠 Collaborating with local organisations to overcome barriers to care ⚽ The link between sports initiatives and health, aimed at improving access to services for underserved populations across Yorkshire and the Humber, is one of the many case studies featured in the report. This initiative sought to address healthcare disparities among underserved groups. By partnering with Yorkshire Sport and their Active Partnerships, the public health team engaged with individuals who typically do not access healthcare services. Integrating health services with sports helped to enhance the accessibility and approachability of health advice. You can read more about this initiative on page 37 of the report.
