Job Summary & Deliverables
The Social Service Worker provides therapeutic behavior management, case management, and other services as defined by licensure to ensure excellence in client care, fiscal management, and regulatory compliance.
Essential Functions
Provides therapeutic behavior management as prescribed by care plan
Assists therapist as needed to develop and review safety/crisis plans and care plans
Develops therapeutic relationships with clients
Reviews and assesses care plans
Coordinates services among internal/external agencies and internal/external providers
Follows care plan to monitor progress, teach skills, address behavioral concerns, and bill accordingly
Ensures continuity of care
Meets required productivity standards and other organizational expectations

Requirements:
Social Service Workers are “the voice”, advocate, and support system for the people and communities that we provide mental and behavioral services for.
Within this role, you will be able to make a difference by assessing client challenges, needs, situations and help develop a support system (treatment and resources) to meet their goals.
For this role and program, we are looking for someone compassionate in listening to, working with, and advocating for those in need.
In addition to sound time management and organization skills, they would need:
Current or soon to acquire SSW licensure within Utah
Bachelors Degree in Social Work or related field
1+ years experience in behavioral health (preferred)
