Senators urge pilot retirement age rise as experience leads to fewer accidents In the letter published on November 20, 2024, lead signee Senator Marsha Blackburn and her colleagues said that data supported the argument that “experienced pilots have fewer accidents than junior pilots”. According to the letter, the ICAO recently “advanced formal action directed at raising or even eliminating the institution’s recommended pilot retirement age of 65 years” at the 14th Air Navigation Conference. OTD in 2014 A catastrophic accident was only prevented by the electrical harness. A series of quality issues were found in the FIPS. This flight crew, everyone on board, and potentially people on the ground were lucky to survive this string of errors. The jump seat pilot criticizes the PF and PM in his report, but what did they do to prevent this near CFIT and near stall on the go around? Here is the excerpt from the NASA report: “We began the approach, but updated weather indicated the airport was below minimums, so we coordinated to hold. While in holding, [Company] advised that another flight landed successfully, and with updated weather, we had the visibility required to begin the approach. ATC amended our holding altitude from 5,000 feet to 7,000 feet, but we forgot to put our new cruise altitude in the Flight Management Computer (FMC) like we did before attempting the first approach. We received vectors to intercept the final approach course and commenced the approach but did not recognize our lack of vertical guidance due to not entering a new cruise altitude. The aircraft appeared to be flying the approach in LNAV/VNAV passing the final approach point, but began a descent rate approaching 1,500 fpm that wasn’t recognized. The Relief Pilot and Pilot Flying (PF) began looking for approach lights as we approached minimums. They had the approach lights in sight, and so we continued the approach, still descending faster than planned. All of us were looking for the runway environment. At about the same time the PF and Relief Pilot saw 4 reds on the PAPI, we received an EGPWS terrain warning. I incorrectly called for a go-around instead of a CFIT (Controlled Flight into Terrain) recovery, and during the maneuver, the pitch attitude became excessive and we received a low airspeed caution as it decreased to around 105 knots. We completed the maneuver, sorted through the distraction of low fuel cautions due to our 10,000 pounds of fuel sloshing during the go-around, and diverted to a nearby airport.” Safety Managers: - How would you investigate this? - How do you prevent this from happening in the future? 🚗 What to Do if Your Car Breaks Down on the Highway in Italy: Keep calm and composed to handle the emergency safely. 📌 Move to the Emergency Lane 📌 Try to reach the emergency lane or a rest stop. 📌 Use hazard lights to signal your maneuver. 📌 If the engine stops suddenly, use the car's momentum to steer to safety, but remember steering will be heavier without power steering. 📌 Signal Your Stopped Vehicle 📌 Apply the handbrake and wear a reflective vest before exiting the vehicle. 📌 Place the emergency triangle about 100 meters behind the car. Turn on your position lights if visibility is low or it's dark. 📌 Contact roadside assistance. 📌 If your phone is dead, walk to the nearest SOS column. 📌 Stay behind the guardrail, away from traffic. 📌 Wait for help inside the car without attempting any repairs. If you’re stranded alone, you might be tempted to hitch a ride from another driver while leaving your car with hazard lights and the emergency triangle in place. However, this is a dangerous mistake and is also illegal under the Road Code (Art. 176 CdS). Attempting to hitch a ride could create a hazardous situation for traffic and increase the risk of an accident. Stay calm, move to the emergency lane, signal your stopped vehicle, and call for help.
