81% of patients (n=83) surveyed is not compliant with the prescribed use of their ankle-foot orthosis. The conference abstract “Ankle-Foot Orthoses Design Shortcomings: User Complaint Survey” by Lennart Zielstra, Irene Beck and Niels Waterval also concluded that the leading complaints causing abandonment were related discomfort and restrictive stiffness. Besides presenting these results, Rezolve joined the panel on patient co-creation during the International Conference for Neurorehabilitation. The main takeaways were: 1) Medical Ethical Committees need to reduce regulatory burdens for low-risk qualitative reasearch to facilitate research-driven patient co-creation (takes 1 year). 2) Patient input is qualitative in nature and is only useful if patients feel free and are able to express themselves. 3) Apart from gaining insights directly from patients clinicians, nurses and physical therapists should be used to efficiently get a grasp of patient problems. 🏆 We are happy to share that ZMD received a grant from the Dick Plettenburg Fund. The start-up is developing the Rise ankle-foot orthosis, which empowers individuals to walk naturally and confidently.   📈 The primary goal of the Dick Plettenburg Fund is to support and accelerate the transition from research to the patient, particularly through an €25.000 grant for (pre-)start-ups. The focus is in the field of treatment and support for individuals with physical or cognitive disabilities.   💪🏽 We wish Lennart Zielstra and Joris Zielstra all the best with their further development of Zeal Mobility and the Rise! What a week it has been! Awesome to come together with other patient innovators during the first week of the “Patient Innovation Bootcamp” which focused on the path of translating our personal experience into rigorous clinical validation. A special thanks to the experts Filipe Costa with his expertise in value based healthcare, João Pinto Coelho sharing his clinical insights, Lúcia Domingues providing insights on the interplay of regulations on reimbursement and last but certainly not least David Siqueira Magboulé for his relentless support for our project!

Bij de keuze van een enkelvoet orthese is het zinvol mee te wegen welk effect de orthese op het gebruik van het hemibeen heeft, niet alleen tijdens het lopen. De keuze van een enkelvoet orthese wordt  zorgvuldig gemaakt. De orthese kan individueel aangepast  worden aan de problemen en mogelijkheden van de patiënt. Er is een ruime keuze.  Ik zie in mijn praktijk echter opvallend veel ortheses met een dikke voetplaat waarbij de  enkel in een vaste stand staat. Een scharnierende orthese is een uitzondering. De consequentie van deze orthesen is, dat bij eenvoudige activiteiten zoals opstaan, gaan zitten, verzitten en traplopen, het hemibeen onvoldoende  gebruikt wordt. De activiteiten kosten meer moeite om uit te voeren. Wanneer de patiënt bijvoorbeeld tijdens het gaan zitten, het gewicht op het hemibeen brengt, ervaart hij zoveel weerstand dat hij automatisch het gewicht naar zijn gezonde been brengt. Het is een uitdaging om bij deze patiënten groep het potentieel eruit te halen. Er wordt aangegeven dat een hoge intensiteit van therapie effectief is. Dit kan je niet alleen bereiken middels oefeningen. Wanneer je het gedrag van de patiënt zo kan beïnvloeden dat hij gedurende de dag meer beweeg momenten heeft, neemt de intensiteit verder toe. Een scharnierende orthese kan de patiënt hierbij helpen.   De ontwikkelingen op het gebied van de orthesen gaat erg snel. Afgelopen jaar heb ik tijdens het IBITA congres in Marsstrand, Zweden een orthese gezien die over de schoen geplaatst wordt (AFO ankle foot orthotics, Best drop foot braces - Turbomed (turbomedorthotics.com). Hoewel dit natuurlijk niet voor iedere patiënt bruikbaar is, waren de collega’s over het gebruik van deze orthese erg positief. Helaas is de orthese nog niet in Nederland te krijgen. Het lijkt me zinvol te onderzoeken wat de consequentie is van een bepaald type enkelvoetorthese op het dagelijks gebruik van het been. Dan kan dit ook meegewogen worden bij de keuze, waarbij de patiënt dat hulpmiddel krijgt wat het beste de revalidatie ondersteunt.
