It Starts With You: A Call to Action for All Physiotherapists As a Council Member for the Physiotherapy Profession under the Ministry of Health Malaysia, I want to emphasize the vital role each of us plays in shaping the future of our profession. This is where Act 774 the Allied Health Professions Act becomes essential. Recent news of a customer becoming paralyzed after receiving chiropractic treatment serves as a stark reminder of the risks posed by unregulated practices. This incident highlights the urgent need for public awareness and protection against unethical and unsafe practices. Act 774 exists to ensure that allied health professionals, including physiotherapists, are qualified, registered, and bound by strict professional standards to protect the public from harm. When a physiotherapist is registered under Act 774, they are legally required to: 1. Practice ethically and within their professional scope. 2. Provide evidence-based care that prioritizes patient safety. 3. Be accountable to regulatory bodies for their actions. Failure to comply with these standards can lead to severe consequences, including suspension of their practicing certificate. To my fellow physiotherapists: I urge you to register under Act 774, regardless of your workplace setting. If you hold an academic degree in physiotherapy, you are required to register. While the registration is currently voluntary, every registration strengthens our profession. A unified profession means a stronger voice, greater recognition, and ultimately, greater progress. Some may ask, “What’s in it for me?” The truth is, the benefits may not seem immediate. But registration isn’t just about you, it’s about the legacy we build together. It’s about ensuring that future generations of physiotherapists have a strong foundation, professional recognition, and a brighter path forward. To the public: Your health is your most valuable asset. When seeking physiotherapy or allied health services, always ensure the practitioner is registered under Act 774. Registered practitioners are qualified, accountable, and legally bound to adhere to professional standards. This safeguards your health and ensures you receive safe, evidence-based care. Choosing an unregistered practitioner exposes you to unnecessary risks, as evidenced by the unfortunate recent case in the news. We don’t need to wait for visionary leaders to guide us. Each of us has the power to be a visionary for the growth of our profession. Your action today could shape the future of physiotherapy in Malaysia. Let’s unite, strengthen our voice, and build a stronger profession. Together, we can leave a lasting impact. Register today under Act 774 because the future of physiotherapy starts with you.

Recently I finished Full Marathon (42km) Kuala Lumpur Standard Chartered Marathon, despite the busy doctoring job, and events being held in the same weekend. Few lessons I learnt for this year FM: 1. Training! Training! Training! -get your muscles ready for exertion that high, I was so ‘busy’ the previous months and weeks, didn’t get proper training, and there are price to pay! 2. Planning -proper hydration, diet, running strategies, including your preparation training. -Also plan specifically: take energy gel at this that km mark, so energy usage will be optimal. 3. Stop and pray! -one of the reason why Full Marathon: so fajr time way past flag-off. Sometimes with flag-off time and we rush to pray, it’ll be inconvenient -I stopped at 19km and took my time to pray, and the first km after fajr prayers is the most energetic run I had, who would expect that? 4. Know your risks: -HR: to ensure a safe run, make sure we run BELOW MAXIMUM (220-age) to reduce risks of tachyarrhythmia. -electrolytes: risks of long run include electrolytes imbalance from out sweat and increased risks of cramps (and subsequently DNF) so we have to ensure proper replacement. I chose chewable salts and no it doesn’t taste as bad as it sounds. 5. Finish strong! Naahh I didn’t do that. Even running towards finishing line is just an act knowing someone was recording me finishing.

It is quite challenging to comprehend that after six years of intensive education, research, and practical experience, graduates of Bachelor of Ayurvedic Medicine and Surgery (BAMS) are regarded as inexperienced, while individuals who falsely claim to be doctors without any formal qualifications receive preferential treatment. I am not sure what people need. It is not necessary for a patient to know the truth from the doctor. A young doctor who reads the report and shares the accurate prognosis of a disease may be perceived as inexperienced. On the other hand, a person who pretends to be a doctor, lacks the ability to analyze a report, and claims that every disease is easily curable, while accepting large payment for such false claims, is preferred. It is rather unfortunate that when the unlicensed medical practitioner was asked about the lack of progress in the patient's condition despite the diagnostic tests, he dismissively remarked that such reports are only relevant to allopathic doctorsAn Ayurvedic physician does not require it. What you are experiencing is the outcome, and the patient foolishly believes it. (After oil massage for a long period give anyone a freshness) Quite a few patients says I don't have any change. Then the fake doctor says you doesn't followed the pathya (diet). Now the patient is convinced , he is right. 🤦‍♂️ People, please understand. Yes, we studied several samhithas, but we are not living in the era of their authors. We are educated enough to read reports. Why digital health? I get this question a LOT. How did I get here. I never intended to become an IT expert, let alone a Digital Health Subject Matter Expert (SME). Dia macam tiba-tiba ehh kat mana ni 😆 No la just kidding (it was a long journey) What I can recall is that, actually all I wanted was to automate my workflows - ESPECIALLY doing roster 🥲 hated it. I felt like my precious time was better spent on treating patients rather than having to stare at the excel spreadsheet calculating hours. And since then, I kept on exploring new things everyday. Many asked me, how can you learn IT? My answer always, "I can ace medical school, learning new stuff is not an issue 😅" Belajar je lah kan? Apa susah hahaha. Through this course, the statistic of the environment could be focused on regarding the implications and solutions. Last week, I had the honor of speaking at the child abuse prevention program organized by the Selangor State Health Department. I discussed the Suspected Child Abuse & Neglect (SCAN) Team, a multidisciplinary group of hospital staff usually led by paediatricians at government hospitals across the country. The team assesses the likelihood of abuse in referred cases and coordinates efforts among various agencies to ensure the safety and protection of the child victims. One of the points, I highlighted was the importance of understanding the strategies used by sexual abusers and pedophiles. These individuals often use enticement rather than entrapment, threats, or force. Since these acts are frequently committed by someone known to the child, the immediate reaction is often confusion rather than fear. As a result, professionals may unintentionally instill guilt in the child when attempting to identify symptoms of Post Traumatic Stress Disorder (PTSD), which may be absent in many young children. (This may appear at a later stage, as young children may be confused and not be able to tell the difference between attention/love and abuse at that time). It’s also crucial to recognize that if a child’s accounts are inconsistent, it indicates both a lack of trust and issue with how information is gathered. When questions probe too deeply into details, children may feel compelled to answer even when they are unsure. This pressure can lead to inaccuracies and further inconsistencies. It is essential for professionals to build trust and ask questions in a manner that allows children to respond comfortably and truthfully. This is an aspect of child abuse intervention that is not commonly discussed but has a significant impact on child victims. We need to realise that how children are treated by the protection team may impact them in a negative way and instill stress, guilt and shame. The reality is, this happens often. By addressing these nuances, we can improve our approach to supporting and protecting vulnerable children.

If you are softworking memang cannot make it I also don’t like it so much when people say they are honest. I need more proof of your honesty. How can you prove it to me? -- I was recently invited by my fellow medical juniors from Universiti Pertahanan Nasional Malaysia (UPNM) to share my experiences from a medical graduate to house officer/medical officer in Ministry Of Health, Malaysia (KKM) up to where I am today at Newcastle University Medicine Malaysia. It was definitely a fun and interactive session with them and I must say that I am quite impressed that they have produced several podcasts at such budding stage of life. Back in the day when I was the 4th medical students batch from UPNM, we did not have much medical related co-curricular activities except for our only Military Medical Students Society (MILIMEDS) and other university based physical related activities. Hence, I was being more active in Malaysian Medical Association, Malaysian Students' Surgical Society (MSSS) and Malaysian Medics International. Good to see on how far the juniors have come. Participation in co-curricular activities is essential for medical students for several reasons: ✔️ Holistic Development: Medical students need a well-rounded skill set that includes communication, teamwork, leadership, and time management, which they can develop through participation in various activities. ✔️ Stress Relief and Well-being: Engaging in co-curricular activities provides students with a much-needed break from their academic rigors, helping to reduce stress and prevent burnout. These can serve as outlets for relaxation and self-expression. ✔️ Networking and Social Skills: Students able to interact with peers, faculty, and professionals from diverse backgrounds. Networking within and outside the medical field can open doors to opportunities for mentorship, research collaborations, and future career prospects while improving on the existing social skills, which are essential for effective communication with patients and colleagues. ✔️ Personal Growth and Self-Reflection: Through participation in co-curricular activities, students can explore their interests, passions, and values outside of medicine. Students can reflect on their experiences, strengths, and areas for improvement, which contributes to their overall self-awareness and development. ✔️ Community Engagement and Service: Medical students have a unique opportunity to apply their knowledge and skills to serve marginalized or underserved populations, promoting health equity and social justice. Amendment of the Medical Act 1971 by the Dewan Rakyat, Parliament of Malaysia 1. Specialist Registration and Training: I have closely followed the ongoing issue regarding the specialist registration, particularly for trainees who have completed parallel pathway specialist training under the Ministry of Health Malaysia. This issue has been under discussion for some time, including during my tenure as Director General of Health and President of the Malaysian Medical Council (MMC). Consequently, I issued a statement on 24 June 2021, following a meeting with the Malaysian Qualifications Agency (MQA) on 15 June 2021. The decision from this meeting was presented and approved at the MMC 407th Meeting on 22 June 2021. The content of that press statement remains accessible on the MMC website. 2. Recognition Process Disputes: Despite thorough deliberations and engagements with relevant parties, as explained in the press statement, the recognition process for international specialist qualifications in the parallel pathway training remains disputed, causing significant discussions within the medical community. Questions and conflicts persist. Thus, the decision by the Minister of Health to consult the Attorney General's Chambers on this matter was both prudent and commendable. 3. Minister of Health's Efforts: I watched the Minister of Health's speech during the second reading of the amendments bill with great interest. I commend Datuk Seri Dr. Dzulkefly Ahmad for his political will to address this critical issue within just six months of his tenure. His efforts have garnered strong support from major professional bodies and associations, indicating thorough engagement and understanding of the technical aspects involved. 1/2

KUALA LUMPUR (Sept 4): IHH Healthcare Bhd (KL:IHH), Malaysia’s biggest hospital operator, said on Wednesday that it is acquiring 600-bed Island Hospital in Penang for RM3.92 billion in cash from Comprehensive Care Sdn Bhd. The owners of Comprehensive Care are: Affinity Equity Partners with 78%, Island Hospital's founder Mark Wee with 16%, plus 41 other individual shareholders who collectively hold 6%. The acquisition includes the vacant land, which is valued at RM223 million with approvals secured for future development, IHH said in a statement. The transaction — to be financed with internal funds and external borrowings — is expected to be accretive to its earnings and return on equity from 2026, the company said. IHH group chief financial officer Dilip Kadambi said the group is expecting Island Hospital to contribute "upwards of 15%" of its total annual revenue. Based on the group's FY2023 revenue of RM20.93 billion, Island Hospital's revenue contribution may come up to over RM3.13 billion annually. Given that we have a lot of hospitals in our network, the revenue contribution from Island [Hospital] wouldn't be so skewed that it will be a big chunk. It will still be upwards of 15% but it is not something which would clearly skew the revenue to one single hospital," he told a virtual press conference. “We expect significant synergies from the transaction and a unique opportunity to elevate Island Hospital as a leading integrated medical facility in Asia,” said IHH chief executive officer Dr Prem Kumar Nair. The acquisition is the latest since March when IHH acquired Timberland Medical Centre for RM245 million, a deal that also came with vacant land in central Kuching earmarked for the construction of a 200-bed tertiary hospital. IHH, controlled by Malaysia's sovereign wealth fund Khazanah Nasional Bhd, operates in 10 countries with Malaysia, Singapore, Türkiye, India and China as key markets. With the addition of Island Hospital, IHH now owns 18 hospitals in Malaysia, including three in Penang, besides Gleneagles Hospital and Pantai Hospital. Once the deal is completed by the year-end, IHH will have more than 1,000 operational beds in Penang. Island Hospital currently has about 119 resident and visiting specialists that attracts over 42,000 inpatient admissions and over 430,000 outpatient visits annually. The average revenue per inpatient admission is RM12,000, according to IHH. The acquisition of Island Hospital is also expected to double IHH's medical tourism revenue in Malaysia, Dilip added. The group claims that over one in three inbound foreign patients to the country uses IHH's network of hospitals.

The Medical Act Amendments concerning specialist training and registration tabled in Parliament by the Health Minister is very progressive and will give flexibility to the Malaysian Medical Council (MMC) to register specialists who meet the required standards and competency. It is what the country needs to meet the shortage of specialists while maintaining standards, safety and quality of care, and is what is done in most other developed countries. The amendments to the medical act addresses the technicalities which prevented the MMC from registering many specialists on the National Specialist Register (NSR), including many of those trained through the parallel pathway - it formalises the role of the Ministry of Health as a specialist training provider and gives the MMC the full authority to recognise specialist training and qualifications if they meet the required standards without the need to refer to any other bodies. https://lnkd.in/ekbfTG48

This upcoming Hari Merdeka has caused me to take pause, and reflect on how it relates to the nation’s healthcare situation today, 67 years after our forefathers won the long and hard fight. Malaysia’s Independence Day, or Hari Merdeka, symbolizes freedom, unity, and resilience. These themes can be effectively correlated with caring for one’s health, especially in the post-COVID era and amidst rising healthcare costs. 1. Freedom Through Health Awareness Just as Malaysians value the freedom gained in 1957, we can value the freedom that comes from being in good health. Taking proactive steps towards a healthy lifestyle—like regular exercise, a balanced diet, and mental well-being—can prevent chronic illnesses, reducing the need for expensive medical treatments. 2. Resilience Against Health Challenges The struggle for independence mirrors the struggle individuals face against health challenges. Post-COVID, the importance of resilience has become even more evident. Independence Day can remind us that just as the nation united and persevered, individuals too must adopt a resilient approach to managing their health, particularly in preventing future pandemics or health crises. 3. Unity in Health and Well-being Merdeka Day emphasizes unity. This can be translated into a collective effort towards healthier lifestyles. By embracing community health initiatives and supporting one another in adopting healthier habits, Malaysians can collectively reduce the strain on the healthcare system. 4. Reducing Healthcare Costs through Prevention Rising healthcare costs can be managed if more emphasis is placed on preventive care, which is a personal responsibility. The spirit of independence can inspire Malaysians to take charge of their health, reducing the reliance on expensive medical interventions. Simple, consistent habits can prevent costly diseases, reflecting the idea that just as the nation built itself after independence, individuals can build a healthier future for themselves. 5. National Health Initiatives as a Form of National Pride Just as the independence movement was a source of national pride, successful public health initiatives can similarly bring a sense of collective achievement. Today marks the end of the June training session. Throughout this training, I have experienced many enlightening moments. Day 1. Morning session (i) The morning session focused on creating our own version of nasi lemak, but the main takeaway was not just about the dish itself. The Graduate Associates (GAs) crafted nasi lemak models using the provided equipment. We experimented with different types of nasi lemak and discussed how to brand our creations to attract potential clients. In Malaysia, there are various types of nasi lemak, each with its own unique twist to appeal to consumers, such as Nasi Lemak Kukus Ayam Berempah with attractive packaging. From this exercise, I realized that the challenge is not just about marketing nasi lemak, but also about marketing ourselves to stakeholders, especially as job seekers. The nasi lemak created by the GAs symbolizes our personal branding. To enhance our marketability, we must develop our own personal brand to attract stakeholders. Just like nasi lemak and its ingredients represent our skills, experiences, and knowledge, the packaging symbolizes our CV, resume, and personal branding efforts. Personal branding is crucial for job seekers to make a lasting impression on stakeholders. One key takeaway from this training is the quote, "If opportunity doesn't knock, build a door." Opportunities are out there, but it's up to us to seize them. Assalamualaikum wr wrb Greetings to everyone !!! Allow me to introduce myself, I am dr. Muhammad Ziaurrahman, a general practitioner who has been working in the field for approximately four years. I have worked in various hospitals, clinics, and even a startup tech company. Throughout my journey and experience as a general practitioner, I have encountered numerous difficulties and heard many complaints from the public regarding the challenges of accessing healthcare services and the unfairness of the healthcare system for the community at present. Not only that, but healthcare professionals including doctors, nurses, midwifes, and all healthcare workers in Indonesia often receive salaries far below the Minimum Wage. With the high demands of the job, and considering the responsibility for patients' lives, healthcare workers receive compensation that is not proportional to their workload and the risks they undertake. Driven by these concerns, some friends and I have taken the initiative to plan and create a public healthcare application that aims to simplify the healthcare system, make it comprehensive, and easily accessible. Our application is called "MEDICO." MEDICO is a digital-based healthcare service application where users can not only engage in teleconsultations with their preferred doctors but also request home visits for direct examinations, ensuring accurate diagnoses and appropriate therapies for patients. Moreover, the healthcare services we provide include nursing care, allowing patients to receive treatment at home, such as intravenous therapy, injection therapy, and various other services, all under the guidance of the doctor who conducted the initial examination. But that's not all; we also dream of providing midwifery services that can directly visit patients' homes for prenatal check-ups and assisting in normal deliveries. We also aim to offer laboratory examination services so that patients being treated at home can undergo lab tests to refine diagnoses and provide appropriate therapy. We plan to provide nutritionist services where experts can visit patients' homes to conduct examinations and create nutrition plans, physiotherapists who can come to patients' homes to provide physiotherapy, and even psychologists who can provide counseling to patients directly at their homes. There are still many more features or healthcare services we can provide. Our goal is to facilitate comprehensive healthcare access and provide fair compensation for healthcare workers. However, we have encountered a hurdle; we do not have connections or individuals interested in investing in our application. Part 1 - Healthcare facilities is nothing without its human resources (HR). When designing a new facility, I have learned that it can not be separated from requesting allocation for HR. But the question is, how much HR is enough? It is apparent in the news that MOH and Malaysia is currently facing challenges in Healthcare HR and it's distribution. The issues before and after Covid has never really died down. Where should we start? There are those who look into developing standards for HR. And I consider myself fortunate to be involve and learn more about it during #nhds study. Healthcare standards are developed to ensure the quality of care through the service it provides. As population grow and age, the quality of care needs to be maintained and monitored, and one aspect is by ensuring the HR is adequate to provide that service. The HR needs to grow in numbers and skill. For that, many professions and divisions have tried to develop their own HR standards to anticipate their future requirement. However, one aspect that I have learned, which the professions have not really considered is, can the space cater for that HR requirement? When we talk about the quality of care, I've learned that we should not just think about how many staff (doctors, dentists, nurses, medical assistants, pharmacists, and other allied healthcare providerd) should be around to care for the increasing number of patients. We should also think about, will the patient be treated in a conducive and safe environment. It is not surprising to see 2-3 doctors sitting back to back in the same consultation room at MOH Health Clinics, when in fact, the building was initially design to cater for 1 doctor per consultation room. This is to ensure privacy to the patient and a conducive environment for the healthcare workers. It is not surprising to see that some hospitals need to close certain wards because the lack of HR to function, while others have patients along the corridors to be treated. Here, I want to highlight that there are two types of HR standards: one according to the facility design, and one according to service requiremenr. The current study that we (Planning Division, MOH) are conducting focuses on the design. - Because the design allows us to plan the HR need from the beginning. The growing HR need based on the services can then be expanded with the expansion of the design. However, we are aware of the challenges to expand, both in service and the physical facility. - Because we need to intervene at a certain point in the vicious cycle of: increasing patient ~ increasing HR demand ~ limited space to provide service. If we don't consider the space, then, not only we are disregarding the privacy and comfort of patients, but also the comfort of healthcare providers to provide a safe and effective care.
