Funds pump record $5.5bn into Indian health in 2023. Private equity and venture capital investment in India's health and pharmlmaceutical sector hit a record of about $5.5bn in 2023, a 25 per cent jump on the previous year, according to a report published by Bain & Company on Thursday. KKR made its latest move in India's rapidly expanding health sector this week, purchasing medical device manufacturer Healthium Medtech from Apax Partners. Advent International last month said it would invest nearly $300mn in a digital and pharmacy arm of Apollo Hospitals, India's largest corporate chain. Government agency Invest India, estimates hospital revenues will hit about $219bn by 2027.

Arisaig Partners slide on India stock market. Sensex over 45 years Vatsal Mody of Arisaig Partners sent me this slide yesterday. The India stock market , ie Sensex has returned close to double digit gains and is slightly higher than the USA stock market over 45 years. My Wharton Professor John Percival had plotted the USA stock market for the first 200 years and the CAGR was 7 pc over 200 years. His thesis was that an efficient,transparent stock market always gives better returns than investment in real estate, in gold, in treasury bonds etc. Only 7 of the original India Sensex are still in the list and four of them are Consumer companies. I believe this is more historical and also a lack of alternative Tech stocks that dominate the USA market. India has the largest number of listed companies in the world, about 5300. The topic of MSME's has been much talked about in India as the backbone of the economy In this context this recent analysis by McKinsey & Company is an eye opener. Some key takeaways: 1. MSME share of business employment is reasonably high at 62% BUT its share of business value add is a dismal 30% - even lower than Nigeria!! 2. Likewise the productivity is low (though higher than Kenya and Nigeria!!) 3. If MSME's could be as productive as larger companies, that would be worth 11% of GDP!!!! 4. Even in per capita terms (discounting for wages), India ranks very poorly in value add and employment share. The question is why? In my opinion it is, once again, because of lack of skill training. What India needs is an apprentice approach like, for example, Germany. It is also because lack of access to funding. Despite the hype around banks supporting MSME's the reality on the ground is very different Lastly I believe MSME's are very, very sub-optimal in scale (an oxymoron) and in many cases VERY manual. The auto OEM's have done a great job in building a trained base of MSME suppliers which could be replicated across other sectors. The latest ‘Healthcare Innovation in India’ report by Bain & Company, in collaboration with HealthQuad, said that healthcare innovation accounts for about 15 per cent ($30 billion) of the overall healthcare market in India, of which around 55 per cent is export-led. One of the things I've mentioned in an earlier post is about executives overstaying their welcome. This applies to boards too. Though on regulatory grounds 2 terms of 5 years each is allowed in India, the point here is that there are cases where there are underperforming board members adding little value. Though this is a global survey by PwC India I wouldn't be surprised if India results would be any different I have seen this first hand where board members are not respected by executive management This is a recipe for disaster. This does not also mean they have to be in cahoots or compliant to each other but mutual respect is a key success factor. #india #corporategovernance #boardofdirectors #directors #governance #executive

MANKIND PHARMA LTD, among India's largest Pharmaceutical companies, is acquiring Bharat Serums and Vaccines Limited (BSV) for a whopping $1.6B, a stellar 15% of Mankind's market cap. Superb exit for PE Firm Advent International which bought the company for $500M in 2020 and successfully turned it around from Rs.800cr in revenues then to Rs. 1723cr in FY24. This deal is interesting but also very strategic - Mankind and BSV share no product overlaps. BSV is a specialized biotech company, R&D-led, with niche and innovative products focused on women's health, assisted reproductive treatments, infertility and critical care. While Mankind is largely a mass-market branded generic product manufacturer (mostly OTC products like anti-biotics, vitamin supplements, pregnancy kits, and for cardiovascular and dermatological care) Some thoughts - ➡ Infertility is a growing concern in India (~25M couples). Rising income levels and the ability to spend on more expensive care treatments makes this a very lucrative vertical. Mankind is strategically positioned now as a significant player here. ➡ I would expect to see more acquisitions by other Indian Big Pharma and MNCs. I suspect they would prefer to acquire biotech companies than have licensing deals and pay in royalties. ▶ Not many innovative molecules in the pipeline today. ▶ The looming global patent cliff-off (when companies lose market exclusivity rights to some of their blockbuster drugs and face competition and price wars from generics) is right around the corner. By 2030, it is expected that $200B of annual revenue sales will be at risk. While this opens a significant growth lever for Indian pharma to partner with MNCs and to solve for affordability and reach of these drugs, pharma revenues globally are expected to stagnate for a while making M&A more attractive. ➡ Hoping for good outcomes for biotech firms with proven science, compelling clinical research trial data and strong technology platforms. The macro-situation would allow for the best firms here to dictate the financial terms in the M&A transaction. Bain Capital has emerged as the frontrunner to acquire a controlling stake in Indian cancer hospital chain Healthcare Global Enterprises Limited. The US private equity firm is considering buying CVC Capital Partners’ 60.4% stake in the Mumbai-listed company, Bloomberg reported. Bain may consider paying a premium to the market price for the stake. Apollo HealthCo Ltd, a subsidiary of Apollo Hospitals Enterprise, has agreed to raise Rs 2,475 crore through an agreement with global private equity firm Advent International. As part of the agreement, Apollo will integrate 100% of its wholesale #pharmaceuticals distributor firm Keimed Pvt Ltd with Advent International over the next 24 to 30 months.
