M&A and turnaround of healthcare facilities!   Took the Masterclass on 28 September.   India is the fastest growing major economy. It is the fifth largest economy and the third largest in terms of PPP. Healthcare industry is one of the biggest employers employing ~7.5 million people. Current size of the healthcare industry is $372 billion and is expected to increase to $638 billion by 2025, growing at a CAGR of 16-17%. India still spends less than 5% of its GDP on healthcare, as against the OECD countries 8-10% and the US a whopping 20%.   Most legacy providers are consolidating aggressively and almost 30000 crores has been invested over the last 4 years with some mega deals have also been signed, powered by PE firms. Part of this investment could be used to extend delivery to Tier2/3 towns wherein the care delivery remains abysmal.   Integration of the newly acquired healthcare facilities (brownfield projects) becomes paramount for the providers/ PE firms to cerate value for their stakeholders. It entails design and execution of a prudent comprehensive business strategy.   Greenfield healthcare projects present a unique challenge to operationalize and turn profitable since all expenses are front loaded. In the absence of comprehensive strategy, turnaround is well-nigh impossible.   I’ve brought out key highlights of the same during my masterclass. In the MedTech startup world, it's not just about creating high-quality medical devices. You also have to tackle challenges like mindset, funding, collaborations, resistance, and regulations. MedTech is a long-term industry, one should not expect quick wins in terms of revenue. Remember, we live in a society where health is not given that much priority until the condition is severe or evident. We need to change how people see MedTech startups in India. Otherwise, we might just become a social venture, and to me there's nothing wrong going this way as well. Kenko Health: From Skyrocketing Growth to Sudden Shutdown Kenko Health’s story was one many startups dream of—rapid growth, big-name investors, and a solution that promised to transform healthcare access. But in a flash, they’ve gone from a company with ₹85 crore in revenue to shutting down operations. What happened? Well, picture this: You’re climbing Everest. You’ve got a great climbing team, amazing resources, and you’ve trained hard. You have got a good chunk of international trainers who have been helping you get there and the local sherpas who are going to provide you with all the necessary equipment to reach the summit. The summit is in sight, but then the sherpas wants the climbing team to be mostly domestic leading to distress among the international trainers who have been helping you reach there. They can't turn their backs now and go back since they are equally invested in making the summit happen. Local Sherpas= Domestic regulator who gives out the coveted Insurance License (summit) International Trainers/ Team Members= Foreign Investors Kenko grew revenue 17x in one year, rising from ₹5 crore in FY22 to ₹85 crore in FY23. Impressive, right? Yet, despite their successes, they stumbled on a critical hurdle: securing an insurance license from the Insurance Regulatory and Development Authority (IRDAI). We all saw the potential. With Lightrock, Peak XV Partners (Sequoia Capital), BEENEXT backing Kenko, the startup seemed destined for greatness. But behind the scenes, things weren’t as perfect. Kenko’s quasi-insurance model worked brilliantly at first, providing affordable subscription health plans. But when it came to the regulatory complexities of acquiring that golden insurance license, things fell apart. They had the customers, the revenue, and the drive, but when the insurance goal became unreachable, the air thinned out. They even had investors lined up for a $27m Series B round—money that would have given them the capital cushion to survive. But then, to receive the insurance license, there were problems. The regulator wanted domestic investors to lead the charge. This, of course, caused a massive restructuring headache. Suddenly, the founders were caught between a rock and a hard place—international VCs on one side, domestic regulatory requirements on the other. And then, the final nail in the coffin—Kenko ran out of money. Even though the founders personally infused ₹9cr to keep the ship afloat, it wasn’t enough. By early 2024, employees weren’t getting paid, debts were mounting, and Kenko’s shutdown was all but inevitable. But this isn’t just about Kenko. It’s a case study on how regulatory hurdles can undo even the most promising companies in the startup ecosystem. Have we achieved healthcare for all or is there still a long way to go? Let's find out the truth 👇🏽    Even though India's healthcare is growing fast, smaller cities still struggle for good care. The critical reasons for this are : 👉🏽 There are fewer specialists in tier-2 & tier-3 cities. This means people still have to travel far for special care. 👉🏽 Many smaller cities don't have technology that is needed for diagnosing diseases. 👉🏽 The government and businesses haven't invested enough in building or improving hospitals in these smaller places. 👉🏽 Many doctors prefer to work in urban areas or abroad because they have more opportunities and patients. This is how we can ensure no Indian is left behind in the pursuit of quality healthcare : ✅ Bigger hospitals can work with smaller ones in smaller cities. This can bring better care and technology to these areas. ✅ Technology companies can help by creating new ways to get healthcare & digital literacy . This can make it easier for people to access care. ✅ The government should spend more money on improving healthcare in smaller cities. This includes building new hospitals and improving the machines they have. ✅ The government should also make sure that healthcare is not too expensive for people in smaller cities. #Take2 Why the Market May Be Overestimating Quick Commerce's Future Despite Damodaran's Optimism As a genuine admirer of #AswathDamodaran and his insightful #valuation frameworks, it's fascinating to see how his evaluations cut to the core of a business's #financial #health. However, with #quickcommerce, it’s possible that the broader market has overestimated the long-term potential of these businesses. Quick commerce is undeniably enticing in terms of #growth and #marketshare grabs, but as Damodaran himself has noted, the sector faces headwinds like narrow #margins and dependency on high-frequency, low-value transactions. Quick commerce relies heavily on the temporary advantage of local #logistics and #labor #arbitrage, but as cities become more congested and labor markets tighten, these operational “advantages” could quickly erode. Furthermore, the political landscape may not always be welcoming, especially given the impact on local #kirana stores, which are integral to India’s #retail fabric. While Damodaran’s framework rightly emphasizes lifecycle positioning, it may not fully capture the #political and #inflationary risks that quick commerce facilitates and faces as it scales. This could mean that valuations, even if optimistic now, might not reflect the long-term stability or profitability the street expects. One critical factor that quick commerce models often overlook is India's regional #diversity —it's anything but a #homogenous market. Local preferences, purchasing behaviors, and infrastructure vary widely from city to city. Quick commerce platforms will need to adapt to these variations, constantly refining and expanding their product categories to cater to local demands. This could mean shifting beyond the basic groceries and essentials model and embracing more regional product mixes, which further adds to complexity and costs. The sector's reliance on ultra-fast delivery and inventory optimization only intensifies these challenges, as what works in a metro city like Delhi or Mumbai may fail in smaller towns where consumer habits and logistical networks differ significantly. This makes scaling in India particularly #complex, and could limit the profitability of the "one-size-fits-all" model that quick commerce often tries to implement. All things considered, I’m confident that these companies have the talent needed to evolve. Just look at how Zomato transformed from its early days to what it is now. The real question is: what will they evolve into, and will that future model justify the premium they currently command? Nothing like meeting a healthtech veteran and winner! Vivek Srivastava and the team at HCAH have created a new industry in India. Home healthcare (literally healthcare at home 😄) is going to be huge. I’ll give you 2 reasons why: 1. Hospitals are now becoming places of serious sick care. Most big hospitals make their money in surgeries and interventions, and they have designed everything from their hiring processes to their support staff to their equipment to their incentives to align with this. 2. Hospitals are themselves carving out separate brands and identities for anything that is not sick care. It stated with Rohit M A and team starting Cloudnine, and now every large hospital has its own post-conception (maybe even pre?) to post-delivery product (Apollo Cradle, Ma Kauvery) etc. But hospitals have the DNA of setting up a shop and treating people who walk in. Setting up a system where care is delivered at home is going to be a whole different ball game. My prediction : anything that is not a serious intervention will be handled at home. It will require some social re-engineering and some bold marketing, and there’ll be lots of chatter about it, but it will happen. And I suspect that the HCAH team will be perfectly positioned to capitalise on that wave 🌊
