Thanks to Temasek, Manipal Hospitals has merged 1200 bedded Medica Hospitals & is now the biggest chain in India with 10,700beds, surpassing Apollo Hospitals (10500beds). The competition is now entering Tier-2 cities!! Medica Superspecialty Hospital, Kolkata ▪ a leading healthcare chain with a total of 11 hospitals. ▫ 60% focus is on Tier-2 cities surrounding Kolkata ▪ Siliguri (WB), Tinsukia (AS), Kalinganagar (OD), Patna (BR), Ranchi (JH) ▫ FY21 Revenue of Rs.0.45Cr/Bed 📈 Is it Temasek’s strategy to take Manipal Hospitals into Tier-2 cities from the EASTERN front of India while all other brands are totally banking on expanding into Tier-2 cities from the NORTHERN side? 🏥 Recently Max Healthcare bought a 550-bed hospital in #Lucknow at a humongous Rs.940Cr i.e. at Rs.1.71Cr/Bed.🤯 💰 Isn't it the price you pay for Tier-1 city hospitals? 😱 Is the premium due to FOMO as Medanta & Apollo both have cemented their positions in Lucknow? 💸 Is it that the market is too huge to earn >30% EBITDA? But I really liked Temasek’s strategy😎: ⬅ It bought 67% of Medica from Quadria Capital at Rs.402Cr, late 2019. ➡ Valuing Medica at Rs.600Cr paying Rs.0.5 to 0.6Cr/Bed 🤑 Giving Quadria a 151% EXIT on Rs.160Cr investment in Dec’13 | CAGR-25.2% 🕴 By 2021 end, It bought more founder shares to own 90% in Medica. Kudos to the founder of Medica Superspecialty Hospital, Kolkata Dr. Alok Roy for envisioning Medica. 🙏 Now, Temasek has MERGED🤝 Medica into Manipal making it the BIGGEST hospital chain in India as per the number of beds 🛏 are concerned!! BUT on ARPOB basis ⬆ Apollo is way ahead at Rs.0.62Cr per bed 🔴Manipal is now at Rs.0.42Cr per bed Even though currently Manipal has become the biggest contender PAN-India basis but the next 3-years (till FY26) are just pure fire🔥 as far as Apollo and Manipal are concerned: 🎾 Apollo will invest Rs.3,435Cr by FY26 to add >2100 beds 🎾 Manipal’ll expand by adding >2000beds by FY26 (Manipal-1400 | Medica-600) In my opinion, the best part is that at least 70% of these 4000+ beds will be based out of Tier-2 cities. PS: Temasek'd purchased 17.74% in Medanta from Punj Lloyd Limited in 2015. Seemingly a structured approach with a clear strategy that immediately makes you believe community provision has real value in the overall system. Out of hospital provision in India has a positive place in the health hierarchy. Will the culture of the UK allow community provision to be reordered in our NHS still dominated by acute organisations and provision? Keep believing.

Agilus Diagnostics, a material subsidiary of Fortis, IHH Healthcare has 31.17% stake in both. As Agilus IPO is postponed, IHH’d buy out the existing PE share to provide them an EXIT, hence controlling more stake in Agilus, the biggest diagnostic company in India. IHH Healthcare is the biggest hospital chain in Asia with 83 hospitals in 10 countries including Singapore, India, Malaysia, China, Hong Kong etc. In 2015, ▪IHH acquired 74.76% stake in Gleneagles Hospitals, India at ▪Rs.1,284Cr paying Rs.1.15Cr/bed for ▪1500 beds in 6-Hospitals in the Southern belt of India. In 2018, ▫IHH won the bidding war against Manipal & TPG & ▫Bought a controlling stake of 31.1% in Fortis Healthcare at ▫A valuation of only $1.1B paying ~Rs.1.9Cr/bed for ▫4600 beds in 34-Hospitals in India & outside. 2019-2023 ▪The revenues of Gleneagles dropped: Rs.1726Cr in '19 to Rs.927Cr in '20 ▪Dr. Ravindranath Kancherla, the owner, dragged IHH to court for 'oppression & mismanagement' eventually winning the cases & sold ▪The remaining stake for Rs.740Cr hence increasing the valuation to ▪Rs.2510Cr i.e. Rs.1.7Cr/bed IHH is happy as this acquisition is just a milestone in its expansion strategy & long-term goals in India: ▫Expand Gleneagles brand in India by ▫Acquiring 250-300 bedded standalone hospitals all over India & ▫Expanding the already acquired individual units further And now it's the 3rd largest in India as well. With this, top-3 positions in India are locked with: 🥇Apollo Hospitals || 10K+ Beds || 73 hospitals 🥈Manipal Hospitals || 10K+ Beds || 33 hospitals 🥉IHH Healthcare || 7K+ Beds || 33 hospitals Now as far as DIAGNOSTIC industry in India is concerned, ▪ India has 100,000 Diagnostic Labs PAN-India. ▫ The top-2 players - Dr Lal PathLabs (~250) & Agilus (~450) claim 0.67% market share ONLY. The market is unbelievably FRAGMENTED!! WHY? Why have the brands NOT been able to expand to say at a level of owning 10,000 Labs all over India?
