FAQ:
1. About permissions(authorization). It can be resolved by:
a. Logout your Google account and login again.
b. Or visit https://myaccount.google.com/permissions , find "Advanced Form Notifcations", then click "Remove access". Return to google form, refresh the page, click the add-on, it will ask to authorize permissions again.

2. About error: drive.google.com refused to connect
It may be caused by multiple signed-in account issues in the browser. To solve it, you could add a new browser profile, and sign in with your account that used to install the add-on (make sure that you haven't signed in with any other accounts) and try to use the add-on.
