During the intake process and annual recertification, we assess the client’s ability to pay a minimal fee based on a share of cost percentage. Assistance with obtaining medications is available through various sources, including the DOH Pharmacy, Patient Assistance Programs (PAP), EAF Prescription Discount Card, and independent pharmaceutical providers offering special rates for EAF clients. We also work with online and local retail pharmacies that offer discounted prescription plans.
Miscellaneous Medical Services:
Our team assists with completing necessary medical forms for non-medical benefits, such as driver’s license applications and SSA forms, to ensure clients have access to all available resources.
Referrals to Other Medical Facilities:
We provide referrals to other medical facilities for services not available at EAF or for non-epilepsy related medical needs, ensuring comprehensive care for all health concerns.
