❗NEW RESEARCH by our own Gizachew Tadele, Nebreed Fesseha, and colleagues: Improving possible serious bacterial infection (PSBI) management in young infants when referral is not feasible: lessons from embedded implementation research in Ethiopia and Kenya  https://rdcu.be/dUHzm RESEARCH HIGHLIGHT: ✴ This multicountry implementation research in Ethiopia and Kenya explores barriers and maps strategies for managing PSBI when referrals are not possible during the COVID-19 pandemic. Despite the varying contexts, common facilitators included community health worker programs and guidelines for PSBI management. Key barriers were community attitudes toward care-seeking, COVID-19 fears, and inadequate healthcare worker competence. Strategies to overcome these barriers involved engaging community health workers, training healthcare staff, implementing COVID-19 safety measures, and advocating for policy integration of PSBI management. ✴ Overall, both projects enhanced PSBI treatment coverage, increased adoption, and informed national policy changes, paving the way for sustainable integration into health systems.JSIDessalew E. ‘Research into antimicrobial resistance in long-term care for people with intellectual disabilities is essential!’ 💊👩‍⚕️ Soemeja Hidad is a PhD candidate at RIVM’s Research, Innovation, and Implementation Department. She conducts her doctoral research at RadboudUMC, focusing on antimicrobial resistance (AMR) and infection prevention in long-term care for people with intellectual disabilities. Soemeja: ‘Most research on antimicrobial resistance has focused on patients in hospitals and elderly residents living in nursing homes. But how significant is this antimicrobial resistance in long-term care settings for people with intellectual disabilities? This remains unknown!’ 🏡 Care in Personal Living Environments ‘People with intellectual disabilities are more likely to have one or more chronic illnesses. Additionally, some live in long-term care facilities and receive daily support and care, increasing the potential risk of spreading resistant bacteria. We must minimise this risk as much as possible. This presents a challenge because infection prevention measures need to be applied in the personal living environments of clients.’ 🚀 A Complete Picture of Challenges and Opportunities ‘We’re not just examining the scale of antimicrobial resistance but also how infection prevention is organised and how antibiotics are used. Quantitative data gives us insight into the prevalence of antimicrobial resistance and use of antibiotic agents while qualitative research helps us understand how infection prevention is experienced and applied in day-to-day care practices.’ 🔬 From Surveys to Wastewater Research ‘Last year, we conducted a survey among professionals. The majority of respondents strongly supported the prevention of AMR and implementing infection prevention measures. However, the active involvement of care facility boards and managers remains essential to drive progress. We are currently setting up wastewater research in long-term care facilities for people with intellectual disabilities. This involves collecting wastewater samples to test for resistant bacteria. Additionally, we are analysing stool samples from clients who use incontinence materials to investigate the prevalence of two specific types of resistant bacteria.’ 👩‍⚕️ Further Research ‘This study represents an important first step in gaining deeper insights. The results will serve as input for more comprehensive follow-up studies. People with intellectual disabilities are often marginalised in society and overlooked when it comes to scientific research, policy-making, and health promotion initiatives. I hope this research helps bridge this gap and makes a small but meaningful step toward driving change.’

Last week Tuesday, my group members and I visited ALAFARA PUBLIC HEALTH CENTER at Ibadan North East Local Government Area, Ibadan. It was a good experience as we interviewed the Health Care Workers on ENAs. Essential Nutrition Actions (ENAs) is one of the topics treated in Community Nutrition.ENAs covers key areas in nutrition hence it importance. What are ENAs? Essential Nutrition Actions (ENAs) are a set of affordable and cost-effective interventions delivered at health facilities and communities to fight malnutrition and improve the growth and micronutrient status of children. The seven Essential Nutrition Actions (ENAs) are: 1. Promotion of exclusive breastfeeding for the first six months of life. 2. Appropriate complementary feeding to support breastfeeding. 3. Feeding of the sick child. 4. Control of anemia. 5. Control of iodine deficiency disorders. 6. Control of vitamin A deficiency. 7. Women's nutrition. #communitynutrition

Ayoola Okuribido, a rising second-year student in the Master of Science in Medical Physics program at the Perelman School of Medicine, will embark on a trip to Ghana this week as part of the Global Medical Physics Training & Development Program (GMPTDP). We had the chance to catch up with him recently to get his thoughts on the opportunity. Here’s some of what he shared: “I jumped at the chance to practice my clinical skills in Ghana because I’m very passionate about global health and healthcare disparities. This program will expose me to what will likely be a very different kind of resource management from what I’ve come to know so far during my education and training. In the process, I expect to learn new approaches to universal issues, which will ultimately help make me a more well-rounded clinician. That tracks with my experience at the #PerelmanSchoolofMedicine so far. I’ve strived to take advantage of as many opportunities as I can, with the aim of expanding my knowledge and enhancing my skill set at each turn. I can’t quite say where I see myself after graduation. I’m keeping an open mind and trying to increase my prospects with each new venture. To me, being prepared for my eventual career doesn’t necessarily mean being homed-in on a particular specialty. New publication on Global Surgery! The Lancet Surgery is typically considered a costly part of health systems that requires advanced technology and is provided in tertiary hospitals. However, most operations are delivered by district and rural hospitals in middle-income countries. Yet there are stark global disparities in surgical outcomes; patients in Africa are twice as likely to die after surgery compared with global averages. An estimated 4·2 million patients worldwide die within 30 days of surgery every year. Combined with constrained health-care budgets, these statistics can lead to a perception that surgical services are low impact and prohibitively expensive. But many surgical procedures are cost-effective. Surgery is integral to high quality care for most non-communicable diseases; cataract surgery restores vision, trauma surgery saves lives and limbs, hernia surgery returns people back to work, cardiac surgery treats ischaemic heart disease, and cancer surgery remains a mainstay of cure for solid tumours. Reframing assumptions by both the surgical community and policy makers about what surgery is and who it is for is crucial, since misconceptions can contribute to inequities in access to essential, life-saving care. The finding by the 2015 Lancet Commission on Global Surgery that 5 billion people do not have access to safe, affordable surgery prompted the 2015 World Health Assembly resolution 68.15, which urged health systems to promote access to emergency and essential surgical care as an essential component of universal health coverage. Since then, National Surgical, Obstetric, and Anaesthesia Plans (NSOAPs) have been developed to scale up surgical provision in 32 countries. Although NSOAPs achieved high-level governmental engagement, many have struggled to identify sustainable funding mechanisms to achieve the expansion of the surgical workforce and surgical facilities needed to meet population needs. Children with TB are often misdiagnosed due to the atypical presentation of symptoms, with about 32% of childhood TB cases in Zambia estimated to be undiagnosed. That is why we have joined forces with the Centre for Infectious Disease Research in Zambia - CIDRZ in the fight against childhood TB and support their aim to increase levels of KAPs regarding childhood TB among healthcare workers. The goal is a high index of suspicion for screening to allow early detection and treatment of TB in children.

In our recent meta-analysis of 11 studies in Uganda (https://lnkd.in/dXqYVxPi), we found that 3 in 10 postpartum women experience depression, with rates even higher in special groups. Despite a slight decrease during COVID-19, prioritizing maternal mental health remains crucial. To improve maternal and child health outcomes, we need to allocate resources for mental health awareness, train healthcare professionals, and establish screening programs for early detection and intervention.
