As a Duty Holder, it's crucial to ensure systems are both compliant and safe. Older systems with hidden elements like fixings or components may not provide the same assurance through typical inspections. To address this, BS 7883:2019 outlines procedures including: ✅ Installation Review (Section 12.2.5): For systems installed before 2019 with incomplete or missing System Technical Files (STFs), this review assesses if they can be updated to meet BS 7883:2019 standards. ✅ Hidden Element Review (Annex J): For systems with concealed fixings or components, this review verifies their condition, design, and installation methods. Hidden elements can be reverse-engineered and, if they meet standards, returned to service. ✅ Document Generation: If systems can comply with BS 7883:2019 but lack necessary documents like drawings or inspection schemes, the STF can be regenerated. It is our duty to uphold these standards, therefore we will be unable to deliver external working at height cleaning and maintenance services if documentation is not up to date.

Based on the information provided, it seems that after the publication of the BS 8000-Part 8 code of practice last year, there were issues identified with the clarity of some drawings and incorrect values for openings in drylining systems. Safety Bulletin: 202010029 Audience: Authorities Having Jurisdiction (AHJs); LPG (Butane/Propane) Extraction Equipment Operators; Type 7 Operators in CA Subject: Vacuum pumps used to pull a vacuum on LPG extraction equipment Priority: High Incident Description: An extraction equipment operator pulled liquid LPG out of the extraction room into an ancillary equipment room through a vacuum line. The liquid LPG ignited in the ancillary equipment room causing the automatic fire suppression system (sprinklers) to activate. No injuries occurred, though there was severe damage to the equipment in the ancillary equipment room and the room itself. Cause: Vacuum pumps are used to remove air from the extraction equipment prior to the introduction of solvent to the system. Many operators use vacuum pumps located outside of the electrically classified area to pull vacuum on the system. In this case, the operator had not fully purged the extraction equipment of LPG from the previous extraction process causing the residual LPG to be pulled through the vacuum pump in the ancillary equipment room. Danger: Fire/Explosion caused by LPG escaping the Class I Division 1 hazardous location boundary. Mitigation/Immediate Action: Vacuum pumps used to remove air prior to the introduction of LPG must be placed within the Class I Division 1 hazardous location. Any electrical vacuum pump used shall be listed by an OSHA approved NRTL and rated for a Class I Division 1 hazardous location. Other options include pneumatic vacuum pumps.

This article presents a probabilistic framework for assessing uncertainty and failure risk in #model-#based #fault #detection (#MBFD) of power electronic systems. The proposed methodology encompasses uncertainty factor selection, uncertainty propagation, risk assessment, sensitivity analysis, and the development of tailored solutions to optimize MBFD performance. By quantifying two types of misdiagnosis, the risk-of-failure of MBFD has been evaluated under diversely random conditions. In a detailed case study on a #modular #multilevel #converter (#MMC), the framework has analyzed five different methods and revealed that existing MBFD methods can have misdiagnosis rates up to 20% due to uncertainties.
