A team from ZHAW Zurich has showcased the transferability of CHO-cell-based mAb production processes from geometrically dissimilar stirred bioreactors for fed-batch and perfusion processes, the team has opened up new possibilities for cost savings and increased yields. Their study found that the high volumetric productivity in the perfusion processes can lead to cost savings, smaller bioreactor sizes, and maintained manufacturing capacity in production plants. Alternatively, yields could be increased by a factor of 2.8 to 3.2 while still using the same bioreactor size. EvlaBio AG is a life science startup dedicated to the development of first-in-class therapeutics in the cardiovascular and cardiorenal space. The lead asset is a therapeutic monoclonal antibody (mAb) targeting the FGF23/FGFR4 pathway for the treatment of left ventricular hypertrophy (LVH) and heart failure with preserved ejection fraction (HFpEF) in the setting of chronic kidney disease (CKD). LVH and HFpEF occur in CKD patients as a consequence of FGF23/FGFR4 overdrive. Hence, blocking the FGF23/FGFR4 interaction has the potential to prevent cardiac hypertrophy and ameliorate heart failure in patients with CKD. The serviceable obtainable market of patients with LVH due to FGF23/FGFR4 overdrive ranges between 5 and 6 M in the seven major markets, representing a substantial commercial opportunity. Based on its underlying mechanism of action, the EvlaBio approach is expected to be hemodynamically neutral. This is a critical differentiating feature from currently available treatment options. With respect to modality, the EvlaBio will be the first biologic developed for this indication, and it has the potential to be a first in class and first in indication therapy. Great initial data from 9 Patients in our phase 1/2 clinical trial of EXG001-307 rAAV gene therapy for young children up to 6 months of age with Spinal Muscular Atrophy (SMA). Improved head control and sitting without external assistance. Highlights: 1. EXG001-307 has a unique engineered AAV design that includes a novel pro-NS promoter, which is believed to contribute to improved efficacy and safety in SMA Type 1. 2. EXG001-307 has demonstrated high expression in target spinal cord tissue and reduced off target expression in liver and heart tissue than AAV9.CBA.SMN1. 3. Patients in the EXG001-307 low-dose group (1.1 E 14 vg/kg) achieved head control 3 to 6 months after dosing and sitting without external assistance 11 months following dosing. 4. The first patient in the mid-dose group (1.5 E 14 vg/kg) achieved sitting with assistance 3 months following dosing. 5. Despite strong foundational research, Germany faces challenges in integrating cell and gene therapies (CGT) into medical care, risking falling behind internationally. A national CGT strategy was presented yesterday.   Here’s how the strategy differs from the CGT strategy in the US and UK:   𝐂𝐞𝐧𝐭𝐫𝐚𝐥𝐢𝐳𝐚𝐭𝐢𝐨𝐧 𝐚𝐭 𝐏𝐚𝐮𝐥-𝐄𝐡𝐫𝐥𝐢𝐜𝐡-𝐈𝐧𝐬𝐭𝐢𝐭𝐮𝐭 (𝐏𝐄𝐈): Germany plans to centralize regulatory processes for GCT at the Paul-Ehrlich-Institut to streamline the regulatory environment. This involves defragmenting the regulatory landscape and introducing "sandbox" procedures as innovation spaces. ​In the US, multiple agencies such as the FDA and NIH handle different aspects of GCT regulation and development, while the UK has the MHRA and other bodies involved in the regulatory process. 𝐆𝐨𝐯𝐞𝐫𝐧𝐚𝐧𝐜𝐞 𝐚𝐧𝐝 𝐒𝐭𝐚𝐤𝐞𝐡𝐨𝐥𝐝𝐞𝐫 𝐈𝐧𝐯𝐨𝐥𝐯𝐞𝐦𝐞𝐧𝐭: Germany emphasizes a "bottom-up" approach involving diverse stakeholders directly in the governance structure. This includes establishing a governance structure with the participation of significant stakeholders and developing a GCT network map and annual progress reports​. This differs from the US and UK, where strategies tend to have more top-down, centralized approaches often led by major government agencies like the FDA in the US or the MHRA in the UK. 𝐅𝐢𝐧𝐚𝐧𝐜𝐢𝐚𝐥 𝐚𝐧𝐝 𝐒𝐭𝐫𝐮𝐜𝐭𝐮𝐫𝐚𝐥 𝐒𝐮𝐩𝐩𝐨𝐫𝐭 𝐟𝐨𝐫 𝐆𝐌𝐏: Germany’s strategy includes specific measures for financial and structural support in GMP production, such as establishing a central GCT-GMP committee and ensuring sufficient financial resources for GMP infrastructures​. This level of detailed infrastructure planning and financial commitment is not as prominently featured in the US and UK. 𝐄𝐦𝐩𝐡𝐚𝐬𝐢𝐬 𝐨𝐧 𝐏𝐨𝐥𝐢𝐭𝐢𝐜𝐚𝐥 𝐄𝐧𝐠𝐚𝐠𝐞𝐦𝐞𝐧𝐭: Germany’s approach includes continuous political engagement and annual progress reporting to ensure political support and adaptation of policies. This structured political integration is more pronounced than in the US and UK, where political support is sought but not as systematically integrated​. 𝐈𝐧𝐭𝐞𝐠𝐫𝐚𝐭𝐢𝐨𝐧 𝐨𝐟 𝐇𝐮𝐦𝐚𝐧𝐢𝐭𝐢𝐞𝐬 𝐚𝐧𝐝 𝐒𝐨𝐜𝐢𝐚𝐥 𝐒𝐜𝐢𝐞𝐧𝐜𝐞𝐬: The strategy highlights the importance of maintaining an open discourse involving humanities and social sciences, ensuring that ethical, legal, and social aspects (ELSA) are considered in GCT development​. This broader societal integration is less emphasized in the US and UK strategies, which focus more on the scientific and regulatory aspects. 𝐌𝐲 𝐭𝐡𝐨𝐮𝐠𝐡𝐭𝐬: I welcome the centralization at PEI. This offers the potential for rapid progress. I have mixed feelings about the other points, as they have the potential to slow things down. What's your thoughts on this?

With the significant reduction of venture capital funding observed over the last two years, it is very important for early stage companies to collaborate with a CDMO aware and adapted to the resulting challenges. Novel operating models enabling timely and cost-effective ways-of-working to progress towards FIH clinical trials are key to permit early stage companies reaching their value inflection points. The ENABLE BIOTECH team has just created such a framework taking into consideration the challenges early stage companies face today.

Exciting Data on Exegenesis Bio's Ocular Capsid to be Presented at ESGCT Rome this Week... Paving the Way for Minimally Invasive Delivery of rAAV-Based Gene Therapies to the Eye! The success of rAAV-based gene therapy for most ocular diseases depends on efficient delivery of the vector to the photoreceptors (PR), and/or retinal pigment Epithelium (RPE). While subretinal administration provides the most direct access to the PR and RPE, it is more invasive, with a risk of damage to the retinal tissue. Intravitreal (IVT) injection is a safe, non-surgical procedure and has been widely used for delivery of protein therapeutics to treat common retinal diseases. Exegensis Bio is currently testing EXG102 in a US-based clinical trial for wet Age-Related Macular Degeneration (wAMD). EXG102 is an rAAV-based gene therapeutic vector expressing a fusion protein that binds VEGF (A/B/C) and Angiopoietin 2 (a novel approach!). This gene therapy is delivered by Subretinal Injection. EXG202 is our next generation gene therapy that uses the same payload, but is packaged in a proprietary, engineered AAV capsid that targets retinal tissues more specifically, enabling delivery via Intravitreal Injection. We think this novel payload, packaged in a highly specific ocular capsid will represent a breakthrough in treatment of wAMD and other common ocular diseases. Over 200 Million people worldwide are living with Age-Related Macular Degeneration or Diabetic Macular Edema. Exegenesis Bio is committed to developing new treatments that address these diseases more effectively by targeting multiple disease pathways and improving delivery to the eye.
