NEW US TASK FORCE RECOMMENDATIONS PUBLISHED APRIL 30TH recommend starting mammograms at age 40. They send a strong message to health care providers that screening should not wait until 50. Here in Canada, the guidelines are expected at the end of May, but we are concerned they will not follow the US and will not recommend lowering the screening age to 40. The new US recommendations REPLACE their 2016 recommendations, which recommended starting mammograms at 50 not 40. WHY DID THE RECOMMENDATIONS CHANGE IN THE USA: The US task force chair stated: “We made this new updated recommendation because the latest science clearly shows that starting at age 40 and obtaining a mammogram every other year until age 74 can further reduce deaths in breast cancer. We can save even more lives — up to nearly 20% more lives — with this updated strategy.” The benefit of screening for breast cancer is that it can help diagnose cases early, before the cancer has spread, which reduces the risk of dying from the disease. The chair also said that the US task force found “clear evidence” that showed that “the incidence of invasive breast cancers in women ages 40 to 49 had been increasing by about 2% every year. And for the first time we also had data on Black women, and we know that Black women are nearly 40% more likely to die from breast cancer compared to White women.” MANY PHYSICIANS ARE CRITICAL OF THE NEW US GUIDELINES AND FEEL THEY STOP SHORT Unfortunately, the US guidelines recommend screening every two years when many more lives would be saved by going annually. According to Dr Wendie Berg, “Annual screening produces greater overall reductions in late-stage disease and deaths due to breast cancer, and greater gains in years of life saved…. Annual screening is particularly important for premenopausal women, especially women in racial and ethnic minority groups.” The guidelines also fall short in these two ways: 1: supplemental screening is not recommended for women with dense breasts. The task force said more evidence is needed. As in Canada, women will need to have a conversation with their provider to determine their personal risk and the type and frequency of breast imaging they need. One size does not fit all. 2: Screening is not recommended over age 74. This ignores women 75+ who are in good health and are at high risk of breast cancer because of their age. In Canada, women can self-refer past 74 in 6 provinces/territories and need a requisition in the others-but they can continue. Poor oral health is an #aging thing. And aging drives the entire healthcare system. In a recent report for the Ontario Hospital Association, it was concluded: "Approximately 1 in 4 adults over the age of 30 will be living with a major illness in 2040, requiring significant hospital care, up from approximately 1 in 8 individuals in 2002. In addition to more people living with major illnesses, the number of illnesses any individual will be living with will also increase significantly with the average number of conditions each person lives with." Projected patterns of illness in Ontario.pdf The implications for the dental industry from an older and sicker population are many, including: 1. fewer visits to the dentist 2. more priority to non-invasive care which can be delivered anywhere 3. an increasing bias to prevention rather than treatment Aging means #Prevora because it is by the far the most preferred and important service to seniors.

Ninety percent of our communication is non-verbal, communicated through the eyes, voice, facial and bodily gestures. From the moment we are born we are communicating our internal cues to our caregivers and we need them to attune to those cues to ensure that we not only survive, but flourish. As developing humans, when we are not consistently cared for or appropriately responded to, when mis-attuned to or neglected, we can suffer relational and development trauma. These developmental gaps and severed neural connections can lead to living in pain, disconnection and chaos. To flourish as humans we need others to co-regulate, co-reflect and to grow brains with –we need this on an ongoing basis for life. It is the same in the psychedelic space - we need others to get our cues and to come into relationship with us when we have dropped into developmental gaps, dissociation, terror and places of thwarted growth. How do we as practitioners intervene wisely during a medicine session? This is a relational art form. In this webinar I share with you, my years of clinical experience within medicine spaces and what it means to facilitate with mutuality, reciprocity and nuanced attunement.

The economic impact of osteoporosis cannot be ignored.    Osteoporosis represents a major global health issue, burdening both individuals and healthcare systems with staggering human and economic costs.    💵 Between Canada, Europe, and the United States, osteoporosis and related fractures are estimated to cost $5 - $6.5 trillion annually [1].    🏥 These costs primarily stem from treating fractures, especially hip fractures, which are associated with high mortality rates – up to 24% within the first year [2].    🩺 The direct medical costs of osteoporosis in the United States alone are projected to be $57 billion dollars [3].     👥 Sufferers often endure a significant loss of independence, with many becoming dependent on care or requiring permanent nursing home admission. This loss of function drastically impacts the quality of life for patients and their families, adding to the human impact of osteoporosis.   📈 The effects of osteoporosis are expected to rise dramatically as populations age. By 2050, the incidence of hip fractures in men is projected to increase by 310% globally [2]. In Europe alone, the cost of fragility fractures is estimated to rise by 27% by 2030 [2]. These figures don't account for indirect costs such as lost productivity or disability.     🟠 Yet, osteoporosis remains widely underdiagnosed and undertreated. CHANGE Do you want change at Doctors of BC? When I joined the board, there’d been years of underinvestment in Doctors of BC. No dues increases for 8 years, no investment, under-valued Physician leaders. Physician leaders operating in Siloes. Strategic plans that valued transforming the system over the needs of physicians. Doctors of BC is about delivering for physicians. This has been my sole focus as a Change-Maker in Doctors of BC. Through my strong leadership within Doctors of BC and in partnership with others I have: ·      Lead a change in the CEO to recruit a moderniser who values and respects uplifting the physician voice. ·      Delivered a PMA with the greatest uplifts for the profession for several years. ·      Started a Governance review:   o   To strengthen the board and the Physician Directors within it – longer terms, more corporate memory. o   Begin the review of the Representative Assembly to better support it in its work. ·      Delivered, in partnership with BC Family Doctors, the Longitudinal Family Physician payment model for Family Doctors. ·      Created a negotiation group for Specialists, in partnership with Consultant Specialists of BC, to negotiate directly with government on remuneration considerations for Waitlist Challenges. ·      Delivered a Strategic Plan that is about you: Uplifting the Physician Voice with plans to ensure you are heard not only with Doctors of BC, but with the government, Health Authorities and the people of BC. ·      Broken down leadership siloes to support the voice of BC Family Doctors, Consultant Specialists of BC and the Joint Clinical Committee Co-Chairs at the Doctors BC board, and in additional spaces to generate new ideas how to work together to support you. I have consistently been a Change-maker throughout my time in Doctors of BC. There is work to be done as big Organisations take time to change. We must invest to: ·      Ensure all our members are seen and heard, not just those in Medical Leadership. ·      Strengthen the Organisation to negotiate effectively, not just the PMA but local contract negotiations that have increased 200% in a short time. ·      Ensure physician leaders are valued and delivering in their roles for you. ·      Improve our communication processes to Improve Transparency of the work we do. ·      Modernise the ways Doctors of BC does it work, by continuing to improve its partnership with the Societies, and providing better support to them. ·      Uplift the work of the Sections by investing in them with better supports to represent their members. ❤️🇨🇦Revolutionizing Ontario Healthcare: Dual Streams for Better Access and Efficiency❤️🇨🇦 What is the bureaucracy waiting for? 🤔 Ontario urgently needs two distinct streams of care within our healthcare system: Physician-led Care” and “Primary Care Practitioner-led Care. Complementary systems that can resolve healthcare issues from beginning to end independently and also in collaboration. Since Bill 179 received royal ascent in 2009, physiotherapists have been waiting for the bureaucracy to update the regulations for OHIP and the Radiology Act! The Legislature did its job in 2009, we need today’s government to tell bureaucrats to get this over the line! These long overdue updates would have made better care a reality for the people of Ontario long ago! When it comes to healthcare, creating bottlenecks doesn’t save money. People will wait for healthcare, so the idea that we can ration healthcare and save money is flawed. Instead, we end up spending more on administrative support, red tape, paperwork, “advisory boards”,“special committees”, “care alliances”, “special networks” and community programs that aren't performed by healthcare professionals and provide absolutely no value to the system. Our Bureaucracy is bringing to life the expression, Penny Wise and Pound Foolish. In the past we created jobs and work that bloat the bureaucracy yet that money is not supporting any direct healthcare delivery to our province. We must accept that there should be TWO different pathways to entering the healthcare system. One for physician related issues and another other serious concerns that can be addressed by physiotherapists, pharmacists, or nurse practitioners. By providing multiple access points to the healthcare system, we reduce the burden on physicians and save money on publicly funded administrative programs that are essentially glorified waiting lists. These programs do not add real value to the overall health and wellness of our province. As Michael Poling highlighted, the real issue isn't a doctor shortage but healthcare funding and fully utilizing the scope of practice of all healthcare professionals. Physiotherapists, for example, should handle MSK issues in ERs and primary care practices, freeing up physicians to focus on their specialties. PTs are also adept at ordering imaging, often more accurately than physicians. However, for this to be effective, the OHIP and Radiology Acts need to be updated to allow physiotherapists to refer directly and the REQUEST to be covered by OHIP. This change would unclog the system and eliminate unnecessary physician referrals. It’s time to update our healthcare system to reflect these realities, streamline care, and ensure better health outcomes for all Ontarians. Minister Sylvia Jones, if you see this, we know that in 2009 you were not the Minister of Health, yet this is something we need you and the Ford government to get done!
