Dharamshila Rahat’s latest OT has been made as per the revised NABH guidelines at an extra cost of several lakhs. It is really disheartening that capital costs of creating a 50-bed high end multispeciality medical centres are skyrocking. No Govt, agency will give you desired certificates /NOC for starting healthcare facility without palm greasing. To cap it all, rates for cashless services being provided by CGHS, ECHS etc. are either not revised for decades and if revised, the rates are very low. The packages rates being offered by insurance/TPA are gradually decreasing. Some of the hospitals have stopped using high quality drugs, disposables, consumables to save costs, which may results in poor patient outcome. We as quality concious people, are not compromising with the quality of equipment, instruments, drugs, disposables and consumables, without worrying about the return on investment.

Defrauding is a strong but an apt word. Breast ultrasound screening centres have made many young unsuspecting women suffer from "pre-existing illness" due to uncovering benign breast cysts and lesions that make them ineligible for breast cancer insurance coverage. Worse when the quality is questionable. Radiologists have to give recommendations on what the images mean. Instead we get a very descriptive result from these ultrasound centres that we have to repeat the ultrasound with breast radiologists that increases the cost of healthcare. Ultrasound breast cancer screening is not evidence based in average risk women. Make sure to have your situation assessed by a breast surgeon to see if you really need an ultrasound. And if you do, make sure you know who is doing the ultrasound. Ultrasound is operator dependant and it's interpretation requires accreditation, high skill and experience.

Fundamental #Math Everyone Must Understand in #Healthcare Testing… Type I and Type II #Errors. Type I Errors are #FalsePositives or What Can be Thought of a Errors of Commission. For Example, a Positive Strep Test in a Child Who Does NOT Have Strep is a Type I Error. Giving that Child Antibiotics is an Error of #Commission. Type II Errors are #FalseNegatives or What Can Be Thought of as Errors of Omission. For Example, a Negative Strep Test in Child Who DOES Have Step is a Type II Error. Not Giving that Child Antibiotics is an Error of #Omission. The Positive and Negative Predictive Value of a Test Can Be Calculated ONLY When You Know the #Specificity and #Sensitivity of a Test AND You Know the #Prevalence of the Disease (e.g. 25% of children with sore throats have Strep or 7% young adults have hypertension). While These Concepts are Somewhat Complicated, They Are Fundamental for Understanding Healthcare. If You Don't Understand These Concepts, It's Easy To Be #Fooled. It's time to take diabetes education beyond city limits and make a meaningful impact in rural areas too. Dear connections Subject - Delay in ECFMG Sponsor notes. I am a medical graduate from Dr. Radhakrishnan Government Medical College , Hamirpur, Himachal Pradesh. My college has applied for ECFMG Sponsor notes in the month of February, 2024 but didn't receive it yet. ECFMG has e-mailed it multiple times that the National Medical Commission has not requested for Government Authority request Info Form from ECFMG and is not replying to their e- mails . We have tried to reach NMC multiple times through phone, emails and visit nothing has yielded any tangible results. what's more sad is that while reaching upon NMC, office we have to wait outside on the road and we were asked to converse through the gaurds, who have no idea what we were talking about. There are more new colleges in the country than ever before be it private or Government and each one lacks ECFMG Sponsor notes. without Sponsor notes no country will accept our application be it USA , Canada , UK Australia, Ireland, Germany, Italy, Switzerland etc. I am sure there are other students outside suffering because of the similar situation. This post is meant to connect with those students and graduates who have given up hopes or they are hoping that they would soon get sponsor notes. Leave the name of your college in the comment section. I want to inform you guys that in India ECFMG sponsor notes are taking an unusually longer time than they should I don't think any of the college started after 2016 has sponsorship. Pakistan's medical council is getting it done within 3-5 months, without the first batch being passed out. It is nothing short of being stripped away from opportunities, we all have passed competitive exams and admissions in government and private colleges but where is that leading us. I would like to share an experience of exemplary interdisciplinary teamwork in management of a very high risk cancer patient at Apollomedics superspeciality hospital, Lucknow. 80/m. CKD V, anuric, on maintenance HD (3/WK). Diagnosed high grade TCC of urinary bladder. After tumor board discussion, he was considered for High risk laparoscopic radical cystoprostatectomy and bilateral ureterostomy. After obtaining cardiology, nephrology and anesthesia clearance (and of course extensive discussion with family), he was finally admitted. Here starts the teamwork. He underwent hd with blood transfusion day prior to surgery under 'nephro-care' (prof Amit gupta & team). Anesthesia given and maintained in steep trendelenberg position by anesthesia team (mind it this is anuric elderly) for about 4 hours (Dr shishir, Dr Meenakshi). Laparoscopic radical cystoprostatectomy with bilateral ureterostomy done by uro team (myself and Dr shailendra), frozen section sent during the case promptly processed by pathology team (Dr kavita, Dr pretty). Patient was reversed out of anesthesia on table itself and shifted to ICU 8 for further care. There he received clinical as well physio care (equally important for recovery without complications) for couple of days (Dr sushil & team, esp. Dr Deepak, Dr Mayank). He was started on orals on day 1 itself. After shifting to ward he was able to get discharged in another couple of days. His final histopathology has revealed T2(a) high grade TCC with no involvement of prostate or ureters with all negative margins. He is adequately palliated and potentially cured. There are too many variables involved in any success, and in any way above names are not exclusive. Team of other doctors, including DNB students, ward and ot nurses, techs, managers, cleaners and above all admin will for excellence with great infrastructure & equipment - when these become one, success happens. We wish to continue to strive for excellence. I ll share the video shortly

Medical Quiz (Real Case)-A 46-year-old male with no co-morbidities, who doesn't smoke and isn't on any medication, presented to the emergency department after experiencing weakness and syncope for one day. The patient had visited a physician earlier, where CBC and RFT were conducted, showing hemoglobin of 14 gm,platelet counts of approximately 2.5 lakhs and TLC of 6000 with normal RFT.In the emergency department, the patient was evaluated and asked to take blood pressure in supine and standing positions, which revealed a significant drop in systolic pressure from 126mmHg in supine to 70mmHg in standing, indicating orthostatic hypotension/syncope. However, the patient was young and healthy,why he will have orthostatic symptoms? So to rule out internal bleeding advised a VBG urgently to check the hemoglobin drop. To my surprise, the hemoglobin level was 18gm, but it helped to clinch the diagnosis. Another test was advised that confirmed the diagnosis. So, what was the diagnosis?
