About us
Your Community Health provides free and affordable medical, dental and allied health services and support programs through a combination of outreach, client-based and centre-based activities. Our purpose is to enable health, wellbeing and dignity for all people in northern Melbourne by providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services. Our services include: - Doctors – bulk billing - Dentists - Counselling - Diabetes Education - Nutrition and Dietetics - Physiotherapists - Podiatrists - Speech Pathologists - Social Support Groups - Men's Shed - Stop Smoking Support - Exercise Groups and Classes - Aboriginal health - Refugee health - Trans and gender diverse health - NDIS - My Aged Care We take pride in delivering a social model of health that meets your individual needs. We provide priority access to people who may face barriers accessing traditional health services. We understand the importance of addressing social, cultural, environmental and economic inequalities through our work. We have three health centres in Darebin and we also provide services in your home or in other community settings across Melbourne's north, such as leisure centres and senior citizen's centres. We are a not-for-profit company limited by guarantee registered as a Community Health Service under Section 48 of the Victorian Health Services Act.

DPV Health is excited to announce our appointment as the lead agency for the North West Melbourne Aged Care Assessment Alliance, delivering the new Single Assessment System for older people in our communities! The North West Melbourne Aged Care Assessment Alliance (NWMACAA) has been appointed by the Federal Government to provide a Single Assessment System (SAS) for older consumers across northern and western Melbourne. The SAS is an end-to-end system to support older consumers to access home care and residential aged care services in a way in which they have greater control and choice in the services they receive. SAS benefits for consumers will include: - Simplified assessment process with the NWM Assessment Alliance having capability to conduct all types of Aged Care Assessments. - Consistent assessments through new streamlined tools. - Wellness and reablement focus allowing people to regain their confidence, ability and the necessary skills to live as independently as possible. - Services will be delivered by local organisations and people representative of the communities they support. The implementation of the SAS follows reforms into Aged Care Assessment arising from recommendations from the Royal Commission in Aged Care Quality and Safety in 2021. DPV Health CEO, Don Tidbury, says “We are delighted to work with our alliance partners to deliver Single Assessment System for older people across Melbourne’s North and West. Currently older people often have to move between assessment agencies as their needs change, doing their best to navigate an overly complex health system. The new streamlined and single system means older people will get supports quicker and more easily with less confusion and duplication.” The Alliance, led by DPV Health, is a consortium of four community health organisations (DPV Health, IPC Health, Merri Health and Your Community Health) and two local councils (City of Darebin and Merri-bek City Council), who are tasked to implement the new system. Unveiled by Minister for Health The Hon Mark Butler MP and Assistant Minister The Hon Ged Kearney MP, this 10-year plan is a huge step towards fair, inclusive, and high-quality healthcare for LGBTIQA+ Australians. “For too long, Australia’s LGBTIQA+ community has faced healthcare barriers that no one should endure – discrimination, misunderstanding, and inequity. That’s why the Albanese Labor Government is launching this first-of-its-kind plan,” said Assistant Minister Ged Kearney MP. This plan is all about changing that. Community Health First’s 2025 Victorian Budget Submission is now live! The submission sets out our plans to scale evidence-based programs that improve health access and outcomes for Victorians and avoid an average of $3 in future healthcare costs for every $1 invested. It includes support for Victorians on public surgery waiting lists, accessible healthcare in regional and rural communities, and preventative services to effectively manage chronic disease outside of hospital.
