Pathways uses an access model of care. What this means is that nurses ensure patients access appropriate care according to their condition, manage waitlists and costs, and give patients the best chance to manage their condition and adhere to treatment. Appropriate referrals make all the difference 💙

There is a multitude of guidelines and regulations that Pathways nurses need to adhere to in the context we work in. Our job behind the scenes is to make sure our nurses have a safe and supportive environment, so that they can deliver services. This means the patient groups we work with don't have to duplicate efforts or build their own internal capacity to support the clinical services provided. Pathways nurses are offered peer support, clinical supervision, training, uniforms, IT support, CPD support including access to Australian Primary Health Care Nurses Association (APNA) membership. We care about our patients as much as we care about maintaining the quality, safety and well being of our nurses.

One of the most important things for our community when accessing allied #HealthServices is finding someone with experience in their condition. This is one of the areas of expertise and responsibility of the #Pathwaysnurse Finding the right allied health care for a patient means they will access the best care and also be more likely to comply with treatment and management. Pathways has moved way beyond health system navigation models of care. The Pathways model focuses on access and this is what a Pathways Specialist Nurse delivers for patients. Pathways specialist nurses work with the patient to identify gaps in care and develop a plan for patients to access the parts of the health system they need.

When you are at #HighRisk of any condition, there is so much #information out there and it can be hard to know what matters in managing your #health. #PathwaysNurses use #evidence-based information to guide their #ClinicalPractice and they share this with their #patients. If you aren't sure of what information is good and what's bad, set an appointment with a #Pathwaysnurse Go to pathwaysnurse.org Genetic Alliance Australia Genetic Support Network of Victoria Pink Hope Scleroderma Australia SATB2 Connect Syndromes Without A Name (SWAN) Australia Pathways Specialist Nurse Service (NGO) Centre For Community-Driven Research (NGO)

#Colds, #flus and other #RespiratoryIllnesses are more common in colder months. Cooler temperatures can also slow #bloodFlow around the body due to constriction of the #veins and #arteries. Extra challenges can arise in cold weather, especially if you have certain chronic conditions. You can book a consultation with a #PathwaysNurse to make sure you are ready for winter. Getting to the point of developing a mental health treatment plan can be a challenging process in itself and patients are often left to navigate wait times, choice of clinician and gap payments. Outside mental health treatment plans, #counselling is a good option for many people. While not usually covered by mental health treatment plans, the cost is often comparable to the gap payment with a psychologist or psychiatrist. There are also #nurse-led interventions that can guide #patients. It's not always straight forward, that's why emotional and mental health is one of the many aspects of #holistic #healthcare that is part of the #PathwaysNurse protocol. Like all #Pathways consultations, a comprehensive consultation is completed where people are supported to access the #HealthSystem and develop #preventative, #urgent care, post-hospitalisation and/or #ChronicCondition care plans, aligned with their health needs.
