Survey Invitation 📝 The Australian Government Department of Health and Aged Care is supporting the development of clinical guidelines for restorative care as part of the new Support at Home (SAH) program. These guidelines aim to inform and enhance multidisciplinary in-home care, with the goal of slowing functional decline and improving outcomes for older adults. ❔ What is Restorative Care? Restorative care is defined as ‘an approach used to delay a person’s need for high-level residential care. Programs are typically short-term and less intensive, costly, and complex than rehabilitation programs, and often take place in community settings.’ (Poulos & Poulos, 2019) 🌟 Your Expertise is Needed! We are seeking valuable insights from professionals like you, who have experience using the Integrated Assessment Tool (IAT), to help identify individuals who may benefit from the restorative care pathway. 🔗 How to Contribute: Please share your valuable feedback by accessing the survey https://zurl.co/YpMT or using the QR code.
